before cancelling the block by
TRD&RVNL.
2. All insulated catenary had been
removed from service in TPJ
division.

that the work was executed in
proper and approved manner.

Guy rod foundation failure

Nature of BWA guy rod foundation top
failure
portion gave away and mast got
bent and BWA rested on the ground.
Cause of There is no proper bonding of
failure foundation due to incorrect mixing
of concrete. Foundation is not of
monolithic in nature.
Remedial 1. TRD staff should involve action
themselves from the foundations
casting stage itself to ensure quality.

failure resulted in reduced clearance due to
less contact wire height at TVR yard.
Continuous spark between OHE and
coal resulted in parting of Contact
wire.
Remedial Leveler gauge has been installed at
action KIKP with a height of 4265 mm.

Non-Fatal Electrical accident

Location 114/45A,ARV YARD VM-KPD Section
Nature of ARV staff met Non-fatal Electrical accident
failure during AOH work.
Cause of Permit to work was availed between ARV/FP-failure
VLR/SP. However The feeder terminated at 114/ 45A is different
section
(ARV/FP-AGM/SP) for which permit to work was not availed.

Remedial Depots were advised to take the Permit to work

for all the OHE, where the staffs were required to
action
work. Proper planning along with power block
requirement must be planned clearly and
explained to staff to ensure staff safety

Nature of BWA guy rod Anchor “V” bolt (RI No 5007-1)
failure cut and counter weight came down and mast
got bent.
Cause of BWA guy rod Anchor "V" bolt (RI No 5007-1)
failure broken due to material failure. Old flaw
observed in the failed specimen. Further
maintenance staff not identified the fault at
the incipient stage, during foot patrolling
due to vegetation growth around the guy
rod area which hindered the proper
visibility.
Remedial
All anchor guy rod loop locations has been
action checked and bushes cleared and ensured its
intactness.

Remedial Safety precaution laid down in ACTM has to be
followed while working on OHE. These
action instructions has been reiterated to all Depots
vide letter [Link]/TPJ/220/OHE
Dated.26.8.22.
Safety seminar also conducted in all depots and
explained about the accident and safety
precaution to be followed during working.
