COVID-19, caused by the novel coronavirus SARS-CoV-2, was
first reported in Wuhan, China, in December 2019. It has
since become a global pandemic, profoundly affecting health
systems, economies, and societies worldwide.

19, including its epidemiology, clinical features,
investigations, management, and prevention strategies.
Epidemiology
•COVID-19 first identified in Wuhan, China, in December

•Transmission: Respiratory droplets, aerosols, and contact
with contaminated surfaces.

in March 2020,
•Impact: High morbidity and mortality, with disproportionate

Epidemiology
The epidemiological triad consists of three components: agent,
host, and environment. Here's how it applies to COVID-19:
AGENT
Causative agent: Severe Acute Respiratory Syndrome
Coronavirus 2 (SARS-CoV-2).
Characteristics: RNA virus belonging to the Coronaviridae
family. High transmissibility with mutations leading to variants
(e.g., Delta, Omicron).
Mode of transmission: Respiratory droplets, aerosols, and
direct contact with contaminated surfaces.
Epidemiology
HOST
Susceptible population: All humans are susceptible, but
certain groups are at higher risk (e.g., elderly,
immunocompromised, individuals with comorbidities like
diabetes or cardiovascular diseases).
Host factors: Immune status, age, sex, and pre-existing
conditions influence disease severity. Behaviors like social
distancing, mask usage, and vaccination affect susceptibility.
Epidemiology
ENVIRONMENT
Environmental factors: Crowded places, poorly ventilated
indoor spaces, and close-contact settings facilitate
transmission.
Seasonal effects: Some evidence suggests higher
transmission in colder months due to indoor crowding and
humidity changes.
Clinical Features
Mild Cases
1. Fever: Usually mild, but can be high in some cases.
2. Cough: Dry or wet cough, which may be mild or severe.
3. Fatigue: Feeling tired or exhausted.
4. Headache: Mild to moderate headache.
5. Sore throat: Mild sore throat.
6. Runny nose: Mild runny nose.
7. Muscle or body aches: Mild muscle or body aches.
Clinical Features
Moderate Cases
1. Pneumonia: Inflammation of the lungs, which can cause
cough, fever, and shortness of breath.
2. Shortness of breath: Feeling winded or having difficulty
breathing.
3. Chest tightness or discomfort: Feeling of tightness or
discomfort in the chest.
4. Confusion or disorientation: Feeling confused or
disoriented.
Clinical Features
Severe Cases
1. Acute respiratory distress syndrome (ARDS): Severe inflammation of the
lungs, which can cause respiratory failure.
2. Respiratory failure: Inability to breathe properly, which can require
mechanical ventilation.
3. Cardiac problems: Heart problems, such as heart failure or arrhythmias.
4. Septic shock: Life-threatening condition that occurs when the body's
response to an infection becomes uncontrolled and causes widespread
inflammation.
5. Multi-organ failure: Failure of multiple organs, such as the kidneys, liver, or
heart.
Asymptomatic Cases are also reported.

Investigations
Molecular Tests:
• - RT-PCR: Gold standard for detecting SARS-CoV-2 RNA.
• - Rapid Antigen Tests: Quick detection of viral proteins
Serological Tests:
• - Antibody Tests: Detect IgM and IgG antibodies.
• - Neutralization Tests: Measure virus-neutralizing antibodies.
Blood and Imaging Studies:
• - Blood Tests: Elevated CRP, D-dimer, lymphopenia,
• - Imaging: Chest X-ray/CT for pneumonia patter
Management
1. Supportive care:
- Oxygen therapy
- Mechanical ventilation if required

2. Medications:
- Antiviral drugs (e.g., Remdesivir)
- Corticosteroids for severe cases

3. Prevention of complications.
Prevention
Vaccination: Complete recommended doses, including boosters.
