Transitioning to Practice 1
12 credit points
Learning outcomes
 Upon completion of this unit, students will be able to: demonstrate integration of knowledge
and clinical skills for safe and effective nursing care;
 demonstrate and critically review the nursing assessments and management for a person
with a range of needs; including recognising, assessing and responding to the deteriorating
patient;
 demonstrate proficiency when performing procedural skills required in the clinical setting;
 identify and demonstrate responsibilities in relation to communication and teamwork for
safe and effective nursing care;
 critically analyse and enact the nurse's legal and ethical role in relation to acute and
complex care with critically ill people or people with acute health episodes; and
 seek and act on feedback about their performance and development as a beginning
registered nurse using the Registered Nurse Standards for Practice Nursing and Midwifery
Board of Australia (NMBA) (2016) as an integrated framework for assessing competence
and determining future development needs.

 Must pass this assessment item to progress to professional experience placement, and
pass the unit. Allowed a second attempt if you fail the first attempt.

 Professional Experience Placement - Relates to Learning Outcomes (LO) LO: 1-6


 Successful completion of 160 hours of professional experience placement.
Must pass this assessment item to pass the unit.
Case Study Report
 Weighted 40% 1500 words LO: 1, 2, 4, 5
 Choose 1 of the 2 case studies provided and answer the associated questions
 Use academic writing with a few exceptions –
- No introduction or conclusion needed, use all of the word count to answer the
questions
- You are required to give an ISBAR handover. Write this as you would say it. No
need for referencing of the handover

 You must demonstrate effective communication skills including AIDET and
ISBAR
 You must demonstrate an A-G assessment
 You must be able to recognize and respond to the deteriorating patient
 You will be asked knowledge questions
 The rubric is available in the assessment section
Clinical Placement
 Placement will be in acute care areas
 160 hours – all hours missed for any reason must be made up
 Consolidation and practice of all clinical skills
 Focus on taking a patient load, time management and priorities of care
 Familiarise yourself with procedures for the deteriorating patient
 Teaching of a colleague

 Please submit books ASAP especially if the placement runs to end of tri
 Make sure you save the document with your name, student number and unit
