URINARY CATHETERIZATION
Lopar
Anatomy of the Urinary System
Introduction
• A specially designed tube into the bladder to drain
urine
• Catheterization is an introduction of a catheter
through the urethra into the urinary bladder
• All catheter care including Catheter insertion is
carried out using an Aseptic Non Touch Technique
(ANTT)
• Catheter - A Tube Used
To Drain Or Inject Fluid
Through A Body
Opening
• Inserted Through The
Urethra, Into The
Bladder To Drain The
Urine.
• Can Be Temporary Or
Left In Place
• A Ballon Is Inflated To
Hold The Catheter In
Place
Types of urinary catheter
• Intermittent catheter- used to drain the bladder
for short period (5-10 minutes)
• Retention/indwelling catheter- this type is placed
into the bladder through a small incision above
the pubic area, it is used for continuous drainage
–Condom catheter
–Silicon catheter
Indications
• To relieve urinary retention
• To obtain a sterile urine specimen
• To facilitate precise measurement of urine
output
• Pre and post-abdominal surgery
• Radiological diagnostic procedures
• Urinary incontinence
• Post prostatectomy and bladder operations
• Too weak patient
• Unconscious patient
• Disabled patients
• Post surgical patient
• Protect wounds or pressure ulcers
• Frequent urinary measurements
Procedure
• Check the manual of clinical procedures in
nursing and midwifery:
1. Assessment
2. Planning
3. Implementation
4. Evaluation
5. Documentation
NURSING CARE FOR PATIENT WITH AN INDWELLING
CATHETER
• Leave the system closed as much as possible
• Do not allow the bag or tubing to touch the
floor
• Always keep the drainage bag below the
level of the bladder
• Keep the catheter and drainage tubing free
of kinks
• Attach the drainage bag to the bedframe
The drainage tubing is coiled on the bed and
clamped to the bottom linen to prevent kinking
of the tubing.
Slack is left on the catheter to prevent pulling.
Notice the
Catheter taped to
The inner thigh.
Notice the drainage
Bag hooked on the
Bedframe.
•The catheter site will need Regular
cleaning to help prevent Infection
•Wear gloves and follow Standard
precautions
•Wash away from the Urinary meatus
• Clean four inches down the catheter
Use a different part of the Washcloth
or a clean antiseptic wipe For each
stroke
• The catheter site will
Need regular cleaning
To prevent infection
• Wear gloves and
follow standard
Precautions
• Clean from the meatus
Down the catheter
• Use a different part
Of the washcloth or
A clean wipe for each
Stroke
EXTERNAL CATHETER
•Also called a:
Condom catheter
Texas catheter
•Used for the
Incontinent male
Patient
•Apply tape in a
Spiral motion
Most facilities change
The external catheter
On a daily basis.
Perform perineal care before
reapplying the external
catheter
Note that the catheter
Is taped to the person’s
Leg to keep it from
Pulling.
EMPTYING THE URINARY DRAINAGE BAG
• Empty the bag at the end of
Each shift
• Measure and record the
Amount of urine present
• Record the amount on the
Intake and output sheet
• Use a graduate to measureThe
amount of urine
• Check the amount of urine in
the Bag at frequent intervals
• Follow standard precautions
and Wear gloves
Catheter associated complications
• Infections (UTI) - can be introduced because of
faulty handling of equipment, breaking the closed
system, or raising the drainage bag above bladder
level – (fluids) Symptoms: loin pain, flank
tenderness, sp discomfort, nausea, vomiting,
confusion
Other complications:
• Leakage/bleeding/blockages
Removal of urinary catheter
• Catheter is removed to facilitate resumption of
normal function of the urinary tract and to
prevent any complications after the desired
therapeutic effect is achieved
Indications for removal
• Routine change of catheter
• Patients who have gained normal bladder
functioning
• Blocked catheter
• Urinary tract infection
Procedure
• Assessment
• Planning
• Implementation
• Evaluation
• Documentation
– Date & time of removal
– Amount, colour, consistency & any abnormality
in the urinary back at the time of removal
– Any specimen collected
– Vital signs
– Any complications and interventions taken
