Chapter 1 – DepEd VMV

Vision Sharing

State how the SPT understands the DepEd VMV

State the SPT’s aspirations for the school and


learners

State the SPT’s roles and responsibilities that will


significantly contribute to the attainment of the
school goals/objectives.
Chapter 2 – Current Situation
Baselines (Where are we now?).

•In this section, discuss the school’s current situation


containing the: status of school performance and list of
alarming data that need to be addressed (refer to Annex 1A –
Annex 2B).
•Discuss also how the school contributes to the overall
performance of the Division using the Gap Analysis Template
(Annex 3).
Chapter 2 – Current Situation
Priority Improvement Areas (What are the urgent
needs?).

•Present the matrix of the identified PIAs as a result of the


prioritization of improvement areas using the criteria set in
Annex 4.

•These PIAs should be aligned to DepEd intermediate


outcomes stated in the Planning Worksheet (Annex 5).

Note: Schools may organize the PIAs using Access,


Quality, & Governance classifications
Chapter 2 – Current Situation
General Objectives

In this chapter, present the objectives that are SMART


and responsive to PIAs, which the school wants to
achieve in Year 1, Year 2, or Year 3 of the current SIP
cycle.
Chapter 2 – Current Situation
Root Causes (What caused the urgent needs?)
•In this chapter, present the ROOT CAUSES of the identified
PIAs in the Planning Worksheet (Annex 5):
•The Rater must check against the documentation of the SIP
process if the SPT really underwent the following processes,
to ensure that the Project Team identify the correct ROOT
CAUSE of an improvement area:
Mapping and analysis of school processes affecting the PIAs
Identification of area of focus from the critical storm clouds within the
mapped process,
Formulation of problem statement

NOTE: Present in this section the completely filled-up Planning Worksheet (Annex 5).
Chapter 3 – Plan

Solutions
•In this section, present the list of solutions for the
identified root cause(s) that were selected based on the
following criteria: within the control of the school,
economical, sustainable, and with support from the
process owners.

•A project design should be developed for each solution


using the format in Annex 9: Project Work Plan and
Budget Matrix. These will not become part of the
Styleguide but will serve as attachments to the Annual
Improvement Plan (AIP).
Chapter 3 – Plan

Project Designs
•The project design for each solution, developed by the
Project Team, should be attached to the Year 1 Annual
Implementation Plan (use Annex 9 or the Project Work
Plan & Budget Matrix). It shall contain the following
information:
Problem Statement (What do we want to address?)
Objective Statement (Where do we want to be?)
Root Cause (What caused the urgent need?)
Activities (How do we get there?)
Timeline (How long will it take?)
Stakeholders (Who are the actors?)
Resources (How much/many do we need?)
Chapter 3 – Plan
Work and Financial Plan or Annual Implementation Plan
It is not necessary to write a paragraph describing the
contents of the WFP or AIP

It is sufficient that the school accomplish the WFP/AIP for


the first year.

WFP format follows Fiscal Year Calendar (plus 1st quarter of


the succeeding FY). It is the basis for Monthly Disbursement
Program and Annual Procurement Plan

Chapter 4

Monitoring and Evaluation

This section contains the plan on how the SPT will


monitor the different Programs, Projects, & Activities or
different areas of school operations. Present the filled-up
Project Monitoring Report Form.
Discuss the roles and responsibilities of the Project
Team and SPT concerning monitoring.
