To: All Department Heads
From: Lisa Irvington, HR Department
Subject: Performance Reviews
Date: October 4

I would like to remind everyone that the performance reviews for your employees need to be submitted no later than
this Friday, October 8 at 5:00 PM. All of them should refer only to the work done by the employees during the third
quarter of this year. [1]
Since a couple of you are doing this for the first time, let me explain the process in brief. You should have already
received the forms for your employees. You need to fill out the blanks on each form. [2]. They need to be
handwritten rather than typed. Simply give your honest opinion of how each individual performed his or her duties
during the month of July, August, and September. If a person did something notable, be sure to include it in the
comments section at the bottom. Finally there is a new question on this form. It asks you if you believe that the
employee should be retained or let go. [3]
If you have any questions regarding the forms, my office door is always open. Or just call me at extension 34, and I
will do my best to answer any inquiries you have. [4] But please don’t wait for the last minute to do this as I am going
to be in meetings all day on Friday and won’t be able to respond to your questions immediately.

154. What is not mentioned about the performance evaluations?

A. They are for only a part of the year.
B. They should be written by hand.
C. They must indicate if the employees should receive raises.
D. They require the people writing them to make comments.
