Level 2: Implementing Permit To Work
• The objective of this training is to describe how the Permit To Work System shall
be implemented within Schlumberger Operations with regard to roles and
responsibilities, subjected activities, work flow and associated documentation.

What is a “Permit To Work” ?
• A Permit To Work (PTW) refers to the “permit” or “work permit”
• A formal document used to communicate work intent and instructions to workers and
management for controls required when working in a potentially hazardous environment
• A written PTW system is an essential part of the management system to help ensure safe
operations
• The PTW is issued before conducting any hazardous work and it:
• Defines the scope of the work to be done
• Identifies hazards and assesses risk
• Establishes control measures to prevent and mitigate risks (through associated HARC)
• Links the work to any other associated work permits
• Is authorized by responsible person(s)
• Ensures adequate control before returning to normal operations

PTW Application
• Activities with a potential risk above acceptable levels require the
implementation of adequate preventive and mitigation measures
• The Permit To Work System shall be applied in the following cases:
• The HARC Analysis conducted for a particular activity defines the Permit to Work System
as an adequate control
• The activities are part of the Segment generic or location specific list of activities
requiring PTW
• The risk associated with activities requires communication and coordination with other
workgroups (Simultaneous Operations ‘SIMOPS’)

How Does PTW Help Manage Risk?

When is a “Permit To Work” Needed?
• When the intended work may adversely affect personnel, the
environment or the site/installation
• Non–routine activities
• Complex multi group activities or SIMOPS (Simultaneous Operations)
• Maintenance work e.g. scaffolding
• On client facilities SLB always work under the client PTW unless the
client does not have a PTW in which case SLB’s PTW will be used
applying only to SLB personnel
• On an IDS/ SPM/ SLR “managed” project, all PTW activities are
managed by IDS/ SPM/ SLR for ALL personnel on site
• PTW is not applied to regular “lower risk” activities – to do so would
weaken the overall effectiveness of the system

Responsibilities
• PTW Holder is the person supervising the job under the PTW
e.g. FSM/ EIC, Crew Chief, Driller, Chief Mechanic, Foreman

• Post the PTW on the PTW notice board
• Review the potential risks and selected controls for the work activities

Work Permit Types
• Hot Work Permit
• All work in hazardous areas if the work involves a potential source of
ignition
• If the task involves use or production of a flame or other ignition
source

Cold Work Permits - Examples
• Issued for work:
• Affecting integrity or • On energized equipment
availability of safety/ (electrical or hydraulic)
emergency systems • Man riding
• Involving pressure testing • Heavy lifts over live
• In confined spaces wells or production
• At heights without guard facility in use
rails • Specialized rigging
• Over water e.g. painting operations (multi crane
over the side of a vessel operations)
• Excavation at (or
adjacent to) well site

Hot Work Permits - Examples
• Issued for work requiring:
• Use of electrical/electronic equipment not certified for
hazardous areas
• Welding and flame cutting
• Opening of live electrical junction boxes
• Grinding
• Use of battery operated camera/ equipment
• Use of air or hydraulic powered metal cutting/chipping tools
• Use of electrical equipment without ATEX certification
(explosion proof) in areas other than the safe zone

Electrical Work - Examples
• A PTW is issued for electrical work:
• When there is potentially hazardous work on any electrical
equipment or electrically driven equipment
• When isolation of high voltage electrical systems, greater
than 1000 VDC or 1500 VAC is required
• When working on high voltage electrical equipment
• When testing high voltage equipment and systems
• When using high voltage portable equipment

The PTW Form
• Key Requirements: EXAMPLE:
• A HARC or a JSA (Job Safety Analysis) must be attached to all
PTWs
• A PTW shall not exceed the duration of a shift
• If personnel change during the job or if conditions change, the
existing PTW must be closed and a new one opened before work
can re-start
• The PTW Holder must check the work area before closing the
PTW
• Active PTWs must be displayed on a PTW board
• PTWs must be filed as per Segment requirements
• If a Segment/location has developed a PTW that meets or
exceeds the requirements in the SLB Guideline then they shall be
allowed to work under their own PTW system

Supplementary Certificates: Isolation
• An Isolation Certificate required when: EXAMPLE:
• The person(s) working on the equipment does not have
sole control of the energy source (s) supplying the
equipment
• When repair, replacement or modification is made to
powered equipment
• When repair or maintenance work requires the removal
or bypassing of equipment protective guards or safety
devices e.g. limit switches, over pressure shut downs etc.
• The isolation certificate shall be raised before a PTW is
issued and shall remain in force until all PTWs associated
to
it are cancelled

Supplementary Certificates: Confined Space Entry

• A Confined Space Entry Certificate: EXAMPLE:

• Is required when it is necessary for personnel to enter


confined spaces including but not limited to; vessels, tanks,
sumps, structural members, enclosed void spaces and other
areas without adequate natural or induced airflow or
breathable air
• Is used to record the certification of atmospheric conditions
inside the confined space through a site gas test and to state
the controls required while work is being carried out
• Does not authorize work to be carried out, an appropriate
PTW shall be approved before work is allowed to start

Other Supplementary Certificates EXAMPLES:
• Based on the type of operations other type of certificates may
be required:
• Excavation/Penetration certificate to ensure:
• The risk of the excavation collapsing is adequately managed
• The risk of colliding with existing piping or other utilities is eliminated
• Vehicle Entry certificate to ensure:
• Any vehicle entering a classified area (Zone 1 and 2 definition of classified
areas as per RMG HSE Manual) at a well site location or a production
facility shall be authorized through a vehicle entry certificate
• Zone 1 in which an explosive gas/mixture is likely to occur in normal
operations
• Zone 2 in which an explosive gas/mixture is not likely to occur in
normal operation but if it occurs, will exist only for a short time

Permit To Work Workflow

PTW Validity
• Cannot exceed the duration of a shift
• Must be clearly indicated on the PTW form
• A PTW is no longer valid when a change occurs in:
• Key personnel performing the work
• Working conditions or conditions that were not anticipated
• When the PTW is closed
• Under no circumstances shall a task be continued after
the corresponding PTW has expired

PTW Expiration and Close Out
• If work is suspended for any reason before completion of the work:
• The area shall be secured and zoned off
• If isolations are in place they shall be maintained
• To resume suspended work a new PTW shall be requested
• Once work is completed:
• PTW holder ensures that equipment and work area are restored to normal working
conditions
• PTW authority gives permissions for any de-isolation to be done
• PTW authority signs off PTW as closed

PTW Display and Filing
• The original copy of the PTW is given to the PTW
holder
• The first copy is posted on the location PTW
notice board
• The second copy is kept by the responsible
supervisor/ Person In Charge of the work
• Upon completion, cancellation or suspension the
original copy is signed off by the PTW authority
• Original PTW are then filed in the location PTW
register with supplementary certificates if any
