1. INTRODUCTION

Emergency response planning is crucial to the level of preparedness to ensure order in


handling of an emergency and promote promptness in the development of the most effective
response.

An emergency is defined as any unplanned event (excluding acts of god and other outside
disasters), requiring the intervention of personnel other than the operations personnel.

These plans have been prepared giving prime consideration to the safety of personnel and
environment on site. The company is mindful of its responsibility to the community and the
environment and as such all on site emergency responses wiII consider their attention.

2. EMERGENCY RESPONSE ORGANIZATION

The Emergency Manager wiII lead the emergency response and has full authority under the
Site Manager. He is supported by personnel on duty with the relevant skiII. All Operators are
trained fire fighters and First Aiders. Some appointed office staff is trained as Fire fighter or
First Aider. As required, he will appoint Fire Fighter or First Aid Team Leaders on the spot.

3. DUTIES AND RESPONSIBILITIES

3.1. Emergency Manager

1. The Emergency Manager is responsible for the control of all operations and the co-
ordination of all emergency response. He evaluates emergency and activates appropriate
level of response, chooses a command post site within view of the emergency and
assembles response support members (as mentioned in Diagram).

He will instruct the departmental responsible and team members on the desired plan of
operations noting the nature of risks associated with the emergency.

In the absence of the Operations Team Leader the Site Manager will occupy this position
and in his absence the most senior operations person on site will occupy this position.

Note:
When the emergency is on a marine vessel or when there is need for the involvement of
marine vessels, there is are no actions needed by the Emergency response organization.
The Emergency manager will inform the state fire brigade.

2. The Emergency Response Manager is responsible for:


 keeping up-to date the Emergency Response Plan and related procedures.
 periodical reviews of the Emergency Response Plan and Emergency Response
Organisation;
 Testing of the Emergency Response Plan. This must be done periodical as described in
the scheme below. Drills may be conducted by individual departments or collectively.
The departmental responsible for the location must report each drill to the Emergence
Response Manager.

3.2. Site Manager


In a major emergency the support and direction of external agencies will be coordinated by the
Site Manager. Equipment and material will be assembled at a staging area and dispatched to
the locations of need as directed by the Emergency Manager. The Site Manager is also
responsible for calIing for additional support requested by the Emergency Manager.

The Site Manager provides support to the Emergency Manager regarding overall strategy for
emergency response and makes decisions regarding financial, statutory' reporting, public
information, legal and contractual arrangements. He will assemble at a safe location remote
from the emergency.

3.3. HSE officer

The HSE officer has two tasks/responsibilities.


1. The Environmental & Safety officer takes necessary process related actions to control the
emergency situation. He marshals personnel in the emergency area and directs initial
response including line isolation, containment or unit shutdown as necessary.
He checks that all personnel are accounted for, initiates search and rescue for any missing
persons and ensures that any required medical attention is obtained.

2. Assess the potential effect of the emergency on the responders, other employees, the
surrounding community and the environment. Makes recommendations strategy for their
protection. He assesses the effectiveness of the response operation and advises. He works
with the entire response teams to devise feasible and safe actions. He identifies sensitive
environmental locations and advises on the best strategy to protect them

As far as is reasonably practicable collects documents data and information with particular
attention to transient information in preparation for the efficient investigation of the incident.
He supplies the Emergency Manager with HSE records and relevant response information.

3.4. Fire Fighting Team Leader


If required the Emergency Manager will appoint the most senior Fire Fighter on duty as Fire
Fighting Team Leader.

The Fire Fighting Team Leader will advise the Emergency Manager about conditions at the
scene including information on combustibles, hazardous materials and associated risks. He will
receive instructions from the Emergency Manager on the desired plan of operation for fighting
the fire.

The Fire Fighting Team Leader controls the Fire Fighters and is directly responsible to the
Emergency manager for accomplishing the plan of action indicated to him. Selection of the
appropriate equipment and methods is the responsibility of the Fire Fighting Team Leader.

3.6. First Aid Team Leader

If required the Emergency Manager will appoint the most senior First Aider on duty as First Aid
Team Leader.
The First Aid Team Leader coordinates First Aid and medical care. In case assistance is
required he will call a Doctor and/or HospitaI and request assistance. He wiIl arrange for
seriously injured patients to be transported to the hospital and coordinate all medical aspects in
respect of fatalities. He is required to maintain records of all injury data and keep the Site and
Emergency Manager informed about injuries and fatalities.

3.7. Emergency Response Team Member

All members of the Emergency Response organization shall endeavour to keep in contact with
the Emergency Manager for issuing advice and reporting or receiving instructions.

3.8. Security Officers

Immediately on hearing the General emergency alarm, Security officers must open any
relevant refinery gates to facilitate access by the emergency site.

It is the responsibility of the Security Officers to prevent unauthorized persons from


endangering themselves by being in a hazardous area. All the access gates must be manned
to prevent looting and other undesired behaviour.

In case employees or members of the public have to be evacuated the Security Officers must
supply appropriate transportation.

3.9. Others
All other personnel on- and off-site shall remain away from the affected area unless specifically
requested to assist.

4. EMERGENCY NOTIFICATION

4.1. Raising the Alarm


When an emergency condition is observed by anyone, his first action must be to raise the
alarm. The alarm can be raised by the following methods:

Manual '"Fire Push Button".


There are manual "Fire Push Button" switches at strategic locations on the site. Raising a flap
and pushing the button activates these. When they are activated the Control Room at the Main
Process Plant is alerted and the sirens in that area sound.

Calling on the Telephone.


Using an internal phone dial the Control Room emergency number.

Control Room Emergency number = 777


When the emergency number is called the following information must be given

"My name is..............:'


Describe the type and location of the emergency.
e.g. "There is a major fire at......."
"There is a toxic gas leak at. ."

Oral Message.
In the absence of a manual fire push button and a telephone at the emergency location, the
warning should be issued orally, ensuring that the message reaches the control room.

On receiving the alarm the Operator on duty will push the General Alarm button at the panel.

4.2. The General Alarm


The General Alarm will be repeated tones of long pulses on the electric fire horns of two
minutes duration.
All Personnel on site and persons living in close" to the refinery will be alerted by the sound of
General Alarm:

4.3. Team Leaders


On instruction from the Operations Team Leader, the operator will call the appropriate
emergency response officer, and page the Emergency Response Team. He must verify that

the message has been sent, by checking the pager assigned to the Control Room.

4.4. Paging the Emergency Response Team


The Emergency Response Team is notified by means of the group paging number: 0777
The activation of the group paging number is responsibility of the Emergency Manager I Site
Manager and/or the Control Room.

To differentiate between the several situations for calls, the following notification codes are
used:

4.5. Security
The Emergency Manager will inform the State Fire Brigade and/or Police and/or the Maritime
Authority.

In case of an emergency all gates must be closed, waiting on instructions of the Emergency
Manager. Individuals will only be left in or outside the plant with his permission.

EMERGENCY RESPONSE PLAN – FIRE / EXPLOSION

1. SCOPE
Fire or explosion on site requires the response of several persons to contain the fire and
mitigate its effect on equipment and the surrounding area. Priority is given to the rescue and
treatment of affected persons while giving consideration to the safety of the responders.

This General Instruction outlines the responses specific to fire and explosion by the site
personnel and the Emergency Response Team detailed in above Chapter - Emergency
Response Plan – General.

2. EMERGENCY RESPONSE ORGANIZATION

Additional to above Chapters of Emergency Response Plan – General.

2.1. Duties and responsibilities .


The Emergency Manager will establish an Emergency Control Point at a convenient location
within view of the fire zone. This point shall be marked by a clearly visible flashing red light.

2.2. Emergency Control Points


The following are pre-arranged Emergency Control Points:
Emergency Control Point 1: East of Main Process Plants
Emergency Control Point 2: East of offices and North of Pit/Oil storage
Emergency Control Point 3: West of Waste Water treatment /South of Boiler house
Emergency Control Point 4: End of factory road/West of Jetty Entrance

2.3. Process Control& Fire Fighting


The Fire fighters will initiate emergency response action bath in respect of making the process
safe as well as carrying out first strike action to control the fire. Fire fighters will approach fire
site from safe direction and conduct fire fight on the instructions of the Emergency Manager.

2.4. Environment
The HSE Officer will conduct an assessment of the degree of risk posed by the event and
advise the Emergency Manager of the necessary action to be taken to avoid harm to
responders, other employees, equipment and the environment.

2.5. Supporting services & Utilities


If the Emergence Manager decides to do so, maintenance wiIl make an assessment of the
need for power isolation, alternative power supply, support transport and lifting equipment etc.
He will report to the Emergency Manager at the appropriate Control Point.

2.6. Security
Security Officers wiIl man the gates to prevent unauthorized entry, make the necessary phone
calls to alert response personnel who are not on the scene.

2.7. External Assistance


The Security Officer will stand by at the Weighbridge building and await the instruction of the
Emergency Manager to call for any external support.

2.8. First Aid


First Aiders will respond to the First Aid Station and await the arrival of any injury.

All members of the ERT (Emergency Response Team) shall endeavor to keep in contact with
the Emergency Manager for issuing advice and reporting or receiving instructions.

All other personnel shall remain away from the affected area unless specifically requested to
assist.

3. EMERGENCY NOTIFICATION

See above Chapter - Emergency Response Plan – General.

4. RESPONSE SEQUENCE

The first responders to a fire / explosion on the site are expected to be the operators since they
are on site continually. Other support groups including the state brigade will respond shortly
after being called. See table 3.

EMERGENCY RESPONSE PLAN - EVACUATION

1. SCOPE
In cases of serious emergencies it may be necessary to evacuate personnel from the site. In
extreme situations the persons living in the community close to the site may also need to be
evacuated. This is a responsibility of the State Fire Brigade or Police.

2. EMERGENCY RESPONSE ORGANIZATION

See above Chapter - Emergency Response Plan – General.

In addition to the Emergency response Team, several persons are assigned as wardens for
selected locations. Wardens and Deputy Wardens are identified with jackets. They must
ensure that all personnel working at their location are aware that they are the wardens for the
location. Wardens, Deputy Wardens and their assigned locations are listed on all information
boards.

2.1. Duties and responsibilities


The Emergency Manager will take overall control of the situation, focusing on the reason for
the evacuation. He is responsible for issuing the evacuation order.
Other members of the Emergency Response Team will take the actions required by the
response plan for the emergency, which demanded the evacuation.

2.2. Wardens
Wardens for buildings and other locations must ensure that these locations are completely
evacuated. The Wardens must report to the Head Warden that all personnel have been
accounted for. The inability to account for the whereabouts of any person who is likely to be in
the area threatened by the emergency must be immediately reported to the Emergency
Manager. The Emergency Manager will direct search and rescue.

2.3. Search & Rescue


The Fire Fighting Captain assisted by Fire Fighters will conduct search and rescue operations
using the appropriate Personal protection equipment and exercising the necessary care to
prevent personal injury.

3. EMERGENCY NOTIFICATION

See above Chapter - Emergency Response Plan – General.

When an evacuation of the site is required the Emergency Manager or his designate will give
the instruction on the radio, telephone-paging system or in person.
The Team Leader or the operator at the control room will sound the General Alarm.

Wardens must ensure that all personnel in their locations are aware of the evacuation order.

4. EVACUATION

All personnel will immediately leave the work area and proceed in an orderly manner via a safe
route and go to the nearest assembly point that is not under threat of the emergency.

Vehicles must be left parked in the parking lots during evacuation. Only emergency response
vehicles may be used.

Muster areas are marked with a green square and escape routes (to the muster areas) are
marked with red lines. See Map next page.

5. ALL CLEAR
Personnel must not return to the evacuated site until the Emergency Manager gives the "All
Clear" sign. This information will be conveyed via the Wardens or Emergency Response
Officers verbal.

6. VISITORS
Visitors are allowed into the refinery to visit specified persons. When an employee has a visitor
he must ensure that he or a designated employee is aware of the whereabouts of the visitor at
all times.
During an evacuation the employee receiving a visitor must ensure that the visitor evacuates to
the assembly point. When the visitor has been passed on to another employee, this employee
is responsible to ensure the evacuation of the visitor.

EMERGENCY RESPONSE PLAN - OIL SPILL


1. SCOPE

The site is designed to capture oil spilled from almost all equipment and direct it to the
collection facilities. Oil spilled outside these facilities is likely to enter the ground water and
waterways resulting in damage to the environment. When there is an oil spill immediate
attention must be paid to containment and clean up. The most significant oil spill event is
spillage into the river or and of the nearby ditches.

2. DEFINITIONS

2.1. Oil Spill stage 1


A Stage 1 Oil Spill is defined as the uncontrolled spillage into the environment of less than 10
barrels of oil that will not enter the public domain or less than one half barrel of oil into the
public domain.

2.2. Oil Spill stage 2


A Stage 2 Oil spill is defined as the uncontrolled spillage into the environment of more than 10
barrels of oil that will not enter the public domain or one half to ten barrels of oil that has
entered or is likely to enter the public domain.

2.3. Oil Spill stage 3


A Stage 3 Oil Spill is defined as the uncontrolled spillage into the environment of more than 10
barrels of oil that has entered or is likely to enter the public domain.

2.4 Oil Spill Response Team (OSRT)


The OSRT is the authority assigned by the organization to coordinate the activities related to a
Stage 3 Oil Spill.

2.5. Environment
Any location not intended to carry or contain oil during its normal usage.

2.6. Public Domain


Any location not owned nor under the control of the organization.

2.7. On-Scene Commander


The official at the scene of the spill has the responsibility for taking action.

3. EMERGENCY RESPONSE ORGANIZATION

See above Chapter - Emergency Response Plan – General.

3.1. Duties and responsibilities


The Emergency Manager will establish an Emergency Management Center at a convenient
location from which he will direct the emergency response. Operations Building or Main
Building may be used for this purpose. Responders must be informed of the selected location.
The location must be equipped with radio and telephone communication. He must be
accessible to squad captains to receive information and issue instructions.

The Site Manager will assemble at the Main Office and will be in contact with the Emergency
Manager via telephone and/or radio.

The Fire Fighting Team Leader will hold the State Fire Brigade on standby at a safe strategy
location in event the oil ignites. If conditions require, the spill may be blanketed with foam.

The HSE Officer will conduct an assessment of the degree of risk posed by the event and
advice the Emergency Manager of the necessary action to be taken to avoid harm to
responders, other employees, equipment and the environment. Special attention must be paid
to the fate of the oil relative to the sensitivity of the locations. He will also advise the
Emergency Manager on the usage of equipment and strategy for clean up.

If the Emergency Manager decides to do so, the Maintenance Manager will make an
assessment of the need for manpower, transportation and lifting equipment for deploying
emergency response equipment.

The Security Officers man the gates to prevent unauthorized entry, make the necessary phone
calls to alert response personnel who are not on the scene.

First Aiders will await a specific call for his response. On arrival he will report to the Emergency
Manager.

All ERT members shall endeavor to keep in contact with the Emergency Manager for issuing
advice, reporting and receiving instructions.
All other personnel shall remain away from the affected area unless specifically requested to
assist.

4. EMERGENCY NOTIFICATION

When an Oil Spill is observed the first action is to inform the Team Leader on duty directly. The
Team Leader will determine the appropriate support systems to be called. '

Note:
On instruction of the HSE Officer the Team Leader will alert the Emergency Response Team
using the paging system.

The Emergency Manager will review the extent of the spill and call out the necessary
assistance. On-site personnel may handle Stage 1 Oil Spills, however when there is need for
additional assistance the Team Leader must not hesitate to call out the relevant personnel.
All Spills must be reported in accordance with the procedure “Reporting of Incidents".
