We, GERYLINE MANALO AQUINO, Filipino citizen, of legal age, and a resident of
BRGY II ALAMINOS, LAGUNA; and NORMA R. MAMBULAO, Filipino citizen, of legal
age, and a resident of BRGY II-D SAN PABLO CITY, LAGUNA, after having been duly sworn
to in accordance with law hereby jointly depose and say that:
1. That we have personally known that Mhark Jhames L. Montano, was born on March 20,
2018 at SPC Regional Hospital;
2. That his/her parents are Mark Anthony P. Montano and Mary Joy B. Lumboy;
3. That we hereby certify, confirm and affirm that the latter above stated name is true and
correct;
4. That we have executed this affidavit for submission to proper authorities and to attest to
the veracity and truthfulness of the foregoing statement and for whatever legal intents and
purposes it may serve.
