IMS4C: THE DESTRUCTION OF MEDICAL RECORDS, DATA AND


INFORMATION IS IN ACCORDANCE WITH THE LAID DOWN PROCEDURE.

Policy: The hospital has a policy for destruction of the medical records after a
designated time
TARGET AUDIENCE: Medical Record Department, IT Department
RESPONSIBILITY: Hospital Administrator, MRD, IT Department

DESCRIPTION:
 The Medical record is destroyed after the period mention above after taking
approval from Medical Director & record is maintained in register.
 Before destroying any records, a notice is advertised in the local newspaper,
stating the details of medical records being destructed. A timeline of seven
days will be given to public in the advertisement in case anyone require the
documents

Procedure:
 Destruction of medical records will be carried out in accordance
with the retention policy of the hospital
 The Medical superintendent/ Director will authorize the
destruction of medical records
 A proper record of all medical records destroyed will be
maintained.
 At the end of Calendar year, a list of UHID.No is taken based on
the Admission date before 5 years (for Non MLC) and permanent
(MLC case). The list contains Patient name and date of Discharge.
 This list is sent to Medical Superintendent for Verification and
Signature of consent for disposal
 After receiving the consent all the case file should be first
shredded by MRD

 A previous day press note should be displayed in the local


newspaper regarding the destruction of medical records along
with time period.
 The shredded pieces should then be burnt by Housekeeping
department
