From Belgian port to English port of dover
Water flooded the lower car deck of the ferry. Cause was the bow door
was left open and this cause the in gush of water. Person incharge of the
door closing was assistant boatswain reason fatigue and sleeping
Squat effect cause the water to gush inside .. In justice sheen report
shore management found neglected many previous unsafe reports of
old masters193 lives lost. Need to address the human element and
failure of irresponsible management.
In October 1989 IMO adopted new guidelines on management for the
safe operation of ships and for pollution prevention. Not compulsory.
Waited for the industry feedback. Found highly useful. So this guideline
was converted into ISM code in November 1993 and were incorporated
as a new chapter 9 in SOLAS in May 1994. Became mandatory from 1 st
July 1998. it was implemented in 2 phases.In india it was implemented in
3phases.

Types of audits
A) For company
1. Interim -when newly registered or taking a new type of
vessel(interim DOC validity 12 months within which the vessel should
acquire vessel and implement ISM )
2. Initial -when we need DOC
3. Annual -every year
4. Renewal -end of 5th year
5. Additional
6. Surveillance audit.-when interim DOC is about to expire, and
company didn’t get the new type vessel, company can apply for a
surveillance audit for again renewing the interim DOC from the date
of expiry. Now it is called as additional audit.
