MEDICAL REPORT
RE: OMORUYI DESTINY /MALE / 24 YEARS
The above-named person is a patient in our hospital; He is known to be suffering from Peptic Ulcer
and Gastrointestinal pain called Cardiomyopathy. The disorder was diagnosed due to the patient
complains of symptoms of Immense Fatigue, Muscular Aches, and Acute Pain in the body system.
Due to this illness situation of extreme physical exertion, immense heat or cold as well as environment
of rigorous activities should be avoided; also, medical monitoring by his physician is essential for the
management of the illness.
In view of the above, the bearer is hereby advised to be coming for medical checkup every fortnightly
appointment. We advised that He should remain within EDO STATE or any nearby STATE, so He
can be better managed by his family and also to keep up with his bi-weekly medical appointment at
the hospital and to avoid any activities that may trigger the symptoms.
DIAGNOSIS: Peptic Ulcer, Cardiomyopathy Muscular Aches and Acute Pain.
