Cholinergic Drugs Classification
A. DIRECT-ACTING (RECEPTOR AGONIST)
- Mimics the effects of acetylcholine by binding directly and activating muscarinic or nicotinic receptors.
Types of Direct Acting
a. Choline Esters
Examples:
Acetylcholine
Bethanechol
Carbachol
Methacholine
b. Cholinomimetic Alkaloids
Examples:
Pilocarpine
Muscarine
Arecoline
B. INDIRECT-ACTING
- Do not act on receptors
- Inhibit the enzyme acetylcholinesterase more Ach available
Types of Indirect Acting
a. Reversible – binds to cholinesterase for a period of minutes to hours
Examples:
Lipid Soluble Water Soluble
Physostigmine Neostigmine
Donepezil Edrophonium
Tacrine Pyridostigmine
b. Irreversible – binds to cholinesterase and form a permanent covalent bond
Examples:
Echothiophate
Parathion
Malathion
Diazinon
Sarin
