DOCUMENT CHECKLIST
WORKERS (APPLICANTS IN CANADA)
This document checklist is part of the forms you need to attach to your application. Refer to the instruction guide.
(IMM 5553) to know if you need to provide some or all of the forms and documents listed in this list of
control. Your request may be denied or sent back to you if required documents are missing. Do not send the documents
originals, unless they are requested from you. The original documents will not be returned to you.

All documents written in a language other than French or English must be translated. Please provide a
photocopy of the document and the translation in French or English.

Important: If you are also applying for a study permit, you must also include the fees and documents.
Requirements for processing your request. Please consult the appropriate guide for the requirements related to students.

Gather your documents in the order indicated in the checklist and check offR each element.

LIST OF FORMS: You must complete, sign, and date the following forms
Application form to change conditions of stay, extend stay, or remain in Canada as a worker (IMM 5710)
Note: If this form has been filled out on the computer, you must validate it to generate a barcode page. Please print and
place the barcode page (page 5 of 5) on top of your request (or on top of each individual request if you
submit a group application
Application for Representation Services (IMM 5476)
Note: Please fill out this form only if you are using the services of an authorized representative, or if you wish to
appoint a representative or revoke the authorization granted to a representative.

Authorization form to disclose personal information to a designated person (IMM 5475)


Note: Please fill out this form only if you authorize Immigration, Refugees and Citizenship Canada (IRCC) and the Agency of
Canada Border Services Agency (CBSA) to share information about your file with someone other than you.

Official declaration form for common-law partnerships (IMM 5409), if applicable

A proof of payment
Note: If you are applying for an open work permit, please provide proof of payment of the applicable fees.

PHOTOCOPIES of the pages of the passport clearly indicating each of the following elements:

the passport number;


the dates of issue and expiration;
the name and the date of birth;
the seal affixed by Canadian authorities during your last entry into Canada; and
all the other marked pages.
PHOTOCOPIES of your travel documents or your identity documents (citizenship certificates, birth certificates, registration certificates)

A PHOTOCOPY of your current immigration document (if you have one)

A PHOTOCOPY of your marriage license or certificate (if applicable)

If you are a dependent of another applicant, please provide the unique client identifier (UCI) of the family member who is
main applicant
If your applicant has obtained a Labour Market Impact Assessment (LMIA) from Employment and Social Development Canada (ESDC), please
provide a copy of the LMIA AND a copy of your job offer letter from your future employer.
If your employer is exempt from the obligation to obtain a Labour Market Impact Assessment (LMIA) from ESDC, they must provide you with the job offer number so that you
you included it in your work permit application form.

If IRCC has authorized your employer to submit the job offer using form IMM 5802, please provide a copy of the IMM 5802 as well as a
proof that your employer has paid the costs related to employer compliance.
Note: If your employer is exempt from the Employer Compliance Program, you must provide a copy of your
employment contract. Learn more about this exemption.

If you are a resident caregiver, please provide a PHOTOCOPY of the signed employment contract between you and your employer.

If you work in the province of Quebec, please provide a Quebec Acceptance Certificate (CAQ) issued by the Ministry of Immigration.

If you are a participant in the International Experience Canada (IEC) program, provide proof of health insurance covering the extension of
the duration of your stay in Canada

IF YOU ARE A STUDENT


Proof of your level of education: a letter from the registrar of the institution and/or photocopies of your transcripts for both
last study periods

IF YOU ARE A CANDIDATE FROM A PROVINCE, YOU MUST PROVIDE


A letter from the provincial government confirming that the province has designated you for the purpose of granting permanent residency.

If your designation has expired, a copy of the acknowledgment confirming that IRCC received your permanent residence application before
the expiration of your designation

A job offer from an employer established in this province (does not apply for applications for open temporary work permits)

A statement from the province specifying the job and the employer (does not apply to applications for transitional open work permits) and
who confirms:
that your employer, who has offered you a job in this province or territory, urgently needs your services;
that the job offer is genuine and that it would lead to advantages or opportunities on the economic front;
that the offered job is neither part-time nor seasonal; and
that the salary and working conditions would be sufficient to attract and retain Canadian citizens in the position in question.

Note: If you have submitted an application for permanent residency, please provide proof that your application has been received and
approved in principle.
If you are the spouse or common-law partner of a holder of an open work permit, that is to say a post-graduate work permit, work permit
working holiday, transitional open work permit under the categories of skilled workers (federal), specialized trades workers
(federal) or family assistance (category 'childcare' or category 'care for individuals with high medical needs'), please
provide
a letter from the current employer of the principal foreign worker confirming employment or a copy of their job offer or employment contract, or a
copy of her job offer or her contract;
a copy of the last three payslips of the main foreign worker and
a copy of the principal foreign worker's open work permit
IF YOU ARE THE SPOUSE OR COMMON-LAW PARTNER OF A HOLDER OF AN OPEN WORK PERMIT OR OF A
SKILLED WORKER
If you are the spouse or common-law partner of a holder of an open work permit, that is, post-graduation work permit, work permit
working holidays, temporary open work permit under the categories of skilled workers (federal), specialized trades workers
(federal) or family caregiving (category 'childcare' or category 'care for individuals with high medical needs'), please
provide
• a letter or a copy of the job offer or contract from the current employer of the principal foreign worker confirming that the job is ...
skill level O, A, or B of the National Occupational Classification (NOC), for the category of skilled trades workers
specialized (federal) level B.
a copy of the last three pay slips of the main foreign worker and
a copy of the open work permit of the primary foreign worker
If you are the spouse or common-law partner of a holder of an open work permit in the Canadian Experience Class category or the category of
candidates from the provinces (regardless of the CNP skill level of the principal applicant), please provide
a copy of the main foreign worker's open work permit
If you are the spouse or common-law partner of a skilled worker who holds a work permit or is authorized to work without a work permit.
under R186, please provide:
• A letter or a copy of the job offer or contract from the main foreign worker's current employer confirming that the employment is of
skill level 0, A or B of the National Occupational Classification (NOC)
A copy of the main foreign worker's work permit
A proof that the principal foreign worker actually resides or plans to actually reside in Canada during their employment.

IF YOU ARE THE SPOUSE OR COMMON-LAW PARTNER OF A FULL-TIME STUDENT


If you are the spouse or common-law partner of a student who holds a study permit, please provide:
• A proof such as a valid enrollment letter demonstrating that the student is enrolled full-time in a Canadian institution such as:
A post-secondary educational institution (for example: university, college, CEGEP, technical or vocational school)
A private post-secondary educational institution in Quebec subject to the same regulations as an institution
public post-secondary
A private or public secondary or post-secondary educational institution in Quebec offering study programs
professional
A Canadian private educational institution authorized to confer degrees.
A copy of the student's study permit

IF YOU ARE AN ASYLUM SEEKER


Proof that you are not able to meet your needs without social assistance (welfare).
If you are currently a social assistance provider, please attach an official letter from the provincial social services stating that
you are a provider or attach a check stub if it clearly indicates that it was issued by the provincial social services.

ET
Proof that your asylum application has been referred to the IRB (Immigration and Refugee Board).
Either of the documents below constitutes acceptable proof:
A PHOTOCOPY of your asylum seeker document,
A PHOTOCOPY of your notice of summons to the ISR or a Confirmation of referred case (CCD).
Note: If you are a citizen of a designated country of origin (POD), you cannot apply for a permit.
work before 180 days have passed since your asylum application was referred to the IRB. (You will be eligible if the IRB
approve your asylum application.

IF YOUR ASYLUM APPLICATION HAS BEEN REJECTED


Proof that you are unable to support yourself without social assistance (welfare).

If you are currently a provider of social assistance, please attach a letter from the social services of the province indicating that you
are you a service provider or attach a check stub if it clearly indicates that it was issued by the provincial social services.
Proof that you have filed an Application for Leave to the Federal Court of Canada and an Application for Judicial Review of the decision
from the CIR in relation to your asylum application.

PROVIDE ONE OF THE FOLLOWING DOCUMENTS (AS APPLICABLE):


A proof that you are appealing the rejection of your asylum application to the Refugee Appeal Section (SAR) of the CISR Commission.

Proof that at least twelve months (or 36 months if you are a national of a designated country of origin) have passed since the rejection of your
asylum application by the CISR

Proof that you are unable to leave Canada as you cannot obtain a travel document.

Examples of documents considered as acceptable proof:


A letter informing you of the rejection of your passport application.
A letter from the Canada Border Services Agency (CBSA) confirming that despite their efforts, they were unable to obtain a
travel title for you.

IF YOU ARE LOOKING TO OBTAIN AN OPEN WORK PERMIT FOR VULNERABLE WORKERS
Evidence that you are a victim of violence or that you risk being one in the context of your employment in Canada.

Evidence of violence or risk of violence may include the following elements:


a description of the violence or risk of violence to which the migrant worker is exposed;
a letter, a statement or a report from a support organization for victims of violence, from a doctor, from a health professional,
or from another entity;
an affidavit from the applicant;
the copy of an official complaint filed with a law enforcement agency:
for example, a police report or from the Canada Border Services Agency, a copy of an official complaint submitted by the
migrant worker with a provincial law enforcement agency, such as a labor standards commission;
supporting or additional documents, such as victim statements, paper copies of emails, photographs
showing injuries or working conditions, testimonies.
