HISTORY OF CHIEF COMPLAINTS: H/C/O Patient complaint of headache present in one side since 10
year which aggrevates while expose to sunlight and cannot tolerate the small sound ,noise and bright
light and headche will continue for 2 days if not taking medicine .
H/C/O Disturbed sleep since 1 year due to work stress .
K/H/C/O Tinnitus since 1 year and it was diagnosed when underwent medical checkup
K/H/C/O Hashimoto’s Thyroiditis since 10 year and under medication .
