Summary of Scenario
Mike is enthusiastic about his position and enjoys learning more about the coding
process. He knows that as he gains experience and earns his certificate, he will be even
more valuable as an employee. As Mike progresses with diagnostic coding, he will also
be able to help the physicians and nursing staff to be attentive to details in
documentation of the patient chart.
Although using the superbill to enter the codes for billing is an easy tool, Mike has
learned that knowing how to use the ICD-9-CM volumes is a necessary asset to ensure
accurate coding. He also knows it is important when coding a diagnosis to ensure the
medical documentation matches the encounter form, and that all elements of the
diagnostic statement are included, and to ensure that the diagnosis listed on the
encounter form is fully documented in the patient’s medical record. In addition, Mike has
learned that the layout and structure of the volumes of the ICD-10-CM manual are
designed to aid in the selection of the most specific and accurate diagnosis code. Every
feature of the manual provides guidance in choosing and confirming a diagnostic code
that matches the diagnostic statement on the encounter form, and in the medical record.
