Pursuant to Division Memorandum No. 380, s. 2025, I respectfully express my intent


to participate in the Pre-Evaluation and Pre-Assessment of Documents for the
Reclassification of Teachers in the elementary.
In line with this, I humbly apply for reclassification to Teacher VI or to any position
that may be deemed appropriate based on my qualifications and submitted
documents.
In compliance with the guidelines stated in the memorandum, I have prepared and
attached herewith the complete set of requirements for the said evaluation, duly
arranged and signed in accordance with the prescribed checklist.
I trust that this request will merit your kind consideration.
