COMMITMENT OF UNDERTAKING

I, CHRISTIAN MELVER BAUTISTA, filipino citizen, of legal age, and resident of PUROK 15
SITIO PUNTOD, SANTOL, BALAYAN, BATANGAS BATANGAS BALAYAN PHILIPPINES
after having sworm to in accordance with law do hereby commit to:

1. Undergo pre-qualifying process in accordance with the industry pre-hiring requirements, if


applicable;
2. Create and maintain a single profile in the Biometric-Enabled Scholarship Registration
System (BSRS), when applicable;
3. Fill up all the items in the Learners Profile correctly and appropriately;
4. Participate actively in the Training Induction of Scholars before the conduct of the training
program;
5. Attend and comply with the required duration of the training program;
6. Refrain from attending any other training/class that will coincide within the training
program;
7. Undergo the mandatory assessment as applicable;
8. Attend graduation ceremonies and other Scholar /TVI activities;
9. Participate actively in the Job Linkaging of the TVET provider, if applicable;
10. Practice good manners and right conduct at all times;
11. Be a member of the TESDA’s Alumni Association; and
12. Provide valid reason/s for dropping out from the training if I would be unable to complete
the training program;

I fully understand that my failure to comply with any of the above may result in my
disqualification as beneficiary of the TESDA's scholarship programs.
