Domestic Vessel: Notice of Arrival (NOA), Application for
Berth/Anchorage (ABA), Vessel’s Roll Book, Passenger Manifest,
Dangerous Cargo Manifest (if applicable), Inward Coasting
Manifest, Waste on Board Vessel Info Form (WOBVIF).

1. Vessel Entrance Formalities Foreign Vessel: NOA, ABA, Passenger Manifest (if applicable),
Dangerous Cargo Manifest (if applicable), Inward Foreign
Manifest, WOBVIF.

Maiden Voyage: Vessel Information Sheet, Certificate of


Inspection (domestic), International Tonnage Certificate (foreign).
Domestic Vessel: Vessel’s Roll Book, Passenger Manifest, Outward
Coasting Manifest, Shore Reception Facility Certificate (if
applicable), Dockage Report, Lifting Order (if applicable),
Computation Sheet/PPA Invoice, Official Receipt of port dues, BIR
Form 2307 (if applicable), PPA Certification for Exemption (if
applicable).
2. Vessel Departure Clearance
Foreign Vessel: Passenger Manifest, Outward Foreign Manifest,
Shore Reception Facility Certificate (if applicable), Dockage
Report, Lifting Order (if applicable), Clearance to Foreign Port
(BOC), Computation Sheet/PPA Invoice, Official Receipt of port
dues, BIR Form 2307, PPA Certification for Exemption (if
applicable).
Domestic Cargoes: Bill of Lading, Cargo Handling/Terminal
3. Entry of Cargoes Operator Receipt, Computation Sheet/PPA Invoice, Official Receipt
of wharfage charges, BIR Form 2307 (if applicable).

4. Withdrawal of Cargoes
Foreign Cargoes: Bill of Lading, Cargo Handling/Terminal Operator

Letter Request, Dangerous Goods Manifest, Inward/Outward
