Speech Intelligibility Rating Scale

This Speech Intelligibility Rating Scale is designed to help speech-language pathologists, educators,
and caregivers assess the clarity of speech in individuals. It provides a systematic method to evaluate
and document speech intelligibility levels, facilitating targeted interventions and monitoring progress
over time.

Rating Scale
Rate the individual's speech according to the following scale, from completely intelligible to completely
unintelligible.

1. Completely Intelligible: Speech is clear and understandable in all contexts, even to unfamiliar
listeners.

2. Mostly Intelligible: Speech is generally clear; occasional misunderstandings may occur with
unfamiliar listeners in complex contexts.
3. Understandable with Effort: Speech is understandable to those familiar with the individual, though
effort is required. Unfamiliar listeners may struggle without additional context.

4. Frequently Unintelligible: Speech is often difficult to understand, even by those familiar with the
individual, particularly in less structured or noisier environments.

5. Completely Unintelligible: Speech is rarely to never understood, even by familiar listeners, across
most contexts.

Instructions for Use

Step 1: Listen to the individual speaking in various contexts (e.g., conversation, reading aloud).
Step 2: Note the clarity of speech and how often the listener understands without needing
clarification.
Step 3: Based on your observations, select the rating that best describes the individual's overall
speech intelligibility.
Step 4: Use the results to identify areas for potential intervention and to track changes in speech
intelligibility over time.
