 Therefore langhana should be done either during the purvarupa stage or the adyavastha.

 Even though langhana stands for ten types of langhana starting from chatusprakara

samsuddhi, here the term langhana more suits for upavasa only.

 Thus langhana is the first line of treatment for jwara rogi, safe guarding the strength with
all efforts ; health depends on strength and the purpose of treatment is maintaining of
health.

BENEFITS

लङ्घनैः क्षपिते दोषे दीप्तेऽग्नौ लाघवे सति ।

स्वास्थ्यं क्षुत्तृड् रुचिः पक्तिर्बलमोजश्च जायते || (अ.हृ.चि.1/3)

When the dosa gets reduced by langhana it helps in

-Agni dipti - Feeling of lightness

-Wellness - Hunger

-Thirst - Desire for food

-Good digestion -Strength and vitality

Contra indication of langhana

 Langhana is not desirable in jwara caused by,

-Vata alone -Kshayaja nidana

-Jirna jwara - Agantuja jwara

 "Suddha vata” here denotes nirama vata. In sama vata also langhana is ideal

VAMANA
 Indication

Vamana is indicated in conditions where,

 The increased dosas have moved out of their seats but not gone out

 Kapha is predominant

 Nausea, increased salivation, aversion to food, cough and visuchika are seen

 Fever manifested immediately after meals

 Sama jwara

 Conditions indicated for vamana

DRUGS

Madanaphala mixed with either pippali, kalinga or madhuka should be consumed either with

honey or with salt followed by drinking of either warm water or water boiled with patola, nimba,

karkota or vetrapatra; or with tarpana (laja) or sugarcane juice or wines

A) KASHYAPANAM

When dosha become pakva by therapies such as langhana, administration of kashya is necessary

either to digest the remaining dosa or to alleviate them.

2. CONDITIONAL TREATMENTS IN JWARA CHIKITSA (BOTH SAMA AND


NIRAMA AVASTHA)

1. Ushna ambu panam

Indication

(अ.हृ.वच.1/23) Vata kapha

 Kindles the digestive activity

 Eases and cleanses the passage

 Initiates the expulsion of stagnated pitta, vata etc

 Dispels sleep, lassitude, and loss of appetite

 Provides vitality

Warm water is contraindicated in cases of,

 Visha and madyaja

 Burning sensation in body conditions

 Jwara with predominance of pitta

 Profound increase of pitta

 Delusion

 Diarrhea

 Kshata kshina

 Raktapitta

 Grishma ritu

Sadanga paniya can be given in such cases.

2. Svedana

Indication

Svedana is ideal in cases of fevers manifesting with,

 Udarda

 Pain in calves, joints, and bones

 Pinasa

 Vata kaphaja fevers

 shwasa

Benefits

Svedana will help in elimination of sweat, urine, faeces and vata and also improves the strength
of the digestive fire.

3. Ghrta panam

Indication

Conditions with less kapha and more of vata and pitta.

 Antagonist to jvara ushma

 Pitta hara due to sheeta guna

 Vata hara due to snigdhatva

 Kapha hara by processing with suitable drugs

Example

Vatika jwara---------->Tilvaka ghrta devoid of trivrita

Paittika jwara----------> Tiktaka ghrtam

Kaphaja jwara----------> Vidangadi ghrtam

4. Sodhana

 If the fever does not subside even with all these therapies then sodhana should be
administered for those who are suitable for them.

 Vamana and virecana can be done according to the dosha. After administration of
sodhana, samsarjana should be adopted commencing with manda, in accordance with
the order.

5. Kshirapanam

Indication

 For those debilitated by jwara, neither vamana nor virecana is suitable. For such a person
expulsion of dosas with either milk or kanaya vasti is suitable.

 Those accustomed to milk

 In whom there is great decrease or loss of sleshma

 Those suffering from thirst and burning sensation

 Increased pitta and vata

 For whom milk is suitable

 Milk is suitable even for patients with diarrhea

 kshira is pathya for both atisara and baddha -vit

Benefits

 It invigorates the body which has become weak by langhana, just as the rain water
helps the forest which has been destroyed by fire. It cures fever soon.

 Hence processed milk consumed as cold or warm or warm milk directly from cows udder,
should be administered to the patient carefully considering the time, otherwise it kills the
patient.

6 Basti prayoga

Niruha basti

When Dosha situated in pakvashaya in pakva avastha

Benefits: Niruha bestows strength

 Relieves fever

 Provides comfort and appetite

 Removes all three mala situated in pakvashaya

Anuvasana basti

Anuvasana can be administered in conditions like,

 Severe depletion of kapha and pitta

 Severe pain in trika, pristha and kati

 Increased digestive fire

 Constipation

6. DHUMA, GANDUSA, KAVALA

Dhuma, gandusa and kavala according to the dosha relieves,

 Bad taste in mouth

7. ABHYANGĀDI PRAYOGA

Abhyanga Alepa------>Jirna jwara localized in skin

Añjana Dhuma-------> Agantuja jwara

SPECIAL TYPES OF JWARA CHIKITSA


1) Sannipata jwara chikitsa.

ििानेनैकदोषस्य क्षपणेनोक्तरि तस्य िा |

कफस्र्थानानुपूव्याा िा तुल्यकक्षाञ्जर्ेन्मलान् || (अ.हृ.वच.1/148)

Using yukti, increasing the dosh that has undergone decrease and decreasing that which has
vitiated and starting treatments initially concentrating on Kaphasthana, the doshas that have
simultaneously vitiated can be subsided.

2) Visama jwara chikitsa

विषमो विषमारम्भवक्रर्ाकालोऽनुषङ्गिान् || (अ.हृ.वन. 2/68)

Visama jwara is irregular in its arambha (commencement of fevers), kriya kala(stages of dosha
vitiation) and continues for a long time.

In the fever that has recurred due to the use of guru, abhisyandi and unwholesome diets,
treatments mentioned above along with langhana and hot therapies (must be performed].

The Punar avartaka Jwara will be alleviated by the regular intake of (decoction) prepared with
kiratatikta, tikta(brhati), musta, parpataka and amrta.

of drugs which are anti febrile

 Mudga, kulattha etc which is light also can be given
