Instructions for Participants:


Please respond to each item by selecting the number that best describes how often you
experience each behavior. There are no right or wrong answers. Be honest and answer
as accurately as possible.

Response Scale:
1 = Not at all
2 = Sometimes
3 = Often
4 = Very Often
5 = Always

Motivational Drive
1. 1. I get started on tasks right away.
2. 2. I have difficulty motivating myself to do things.
3. 3. I put off doing things I should do.
4. 4. I need strong incentives to get started on something.
5. 5. I often lose interest quickly.

Organization
6. 6. I plan out my day.
7. 7. I keep things where I can find them.
8. 8. I organize my time well.
9. 9. I use reminders or schedules.
10. 10. I often misplace things.

Strategic Planning
11. 11. I think ahead before acting.
12. 12. I consider different ways to solve a problem.
13. 13. I weigh the pros and cons before making decisions.
14. 14. I make plans and follow through with them.
15. 15. I set goals and work toward them.

Impulse Control
16. 16. I act without thinking.
17. 17. I say things without considering the impact.
18. 18. I get in trouble for speaking or acting impulsively.
19. 19. I interrupt others when they are talking.
20. 20. I have trouble waiting my turn.

Empathy
21. 21. I can tell how others feel.
22. 22. I am sensitive to others’ moods.
23. 23. I care about how my actions affect others.
24. 24. I try to see things from another’s perspective.
25. 25. I notice if someone is uncomfortable.
26. 26. I often know what people need without them saying.
27. 27. I try to help others when they are struggling.

Scoring Guidelines
Reverse-score the following items: 2, 3, 4, 5, 10, 16, 17, 18, 19, 20 (e.g., a "5" becomes
"1", a "4" becomes "2", etc.).
Sum each subscale (5 items each except Empathy with 7).
Higher scores = Better executive functioning.
