As per group policy, in line with our commitment to the health and safety of our colleagues, facilities for the annual Periodic Medical Examination (PME) are being
made available pan India for all employees
You are required to undergo a periodic medical check-up every year. Your medical check-up is due in this month.
The details of the tests required to be done are as follows:
Category B: Age 40 years & above
1. CBC with ESR
2. Urine Routine
3. Blood Sugar - Fasting & PP
4. TSH
5. Serum Ceratinine
6. Uric Acid
7. BUN
8. Lipid Profile
9. Liver Profile
10. Prostate Specific Antigen (for males)
11. ECG
12. Chest X-ray
13. Spirometry/PFT
14. Stress Test (2D ECHO- if Stress Test is contraindicated)
15. Gynaec Examination with PAP Smear (for females)
16. Mammography (Every alternate year) (for females)
17. Ultra Sonography (USG) Abdomen & Pelvis
18. Family History: HT /DM / IHD
19. Personal History (Past): HT / DM / IHD
20. Personal History: Alcohol-Y/N, Tobacco-Y/N, Smoking-Y/N, Exercise : Regular Y/N; Any other relevant history.
21. General Examination (Height in cms, Weight in kgs, Pulse, Blood Pressure)
22. Systemic examination - RS, CVS, CNS, PA with Clinical breast examination
23. Eye Check Up (by eye specialist)
You may choose to undergo the same at any of the empaneled centers where you will not be required to make any payment for the Checkup. You can check the
list of empaneled centers by following the below steps,

You need to take prior appointment for health checkup.
Please carry your Company Identity Card along with a printout of this letter when you go for PME checkup. Hospital will directly send your check-up reports to the
he Group Medical Advisor's office at Reliance Industries Limited, Maker Chamber-IV, 2nd Floor, West Wing, Nariman Point, Mumbai-400021. The same will be
made available in your Health Locker on JioHealthHub.
*(Attaching PME letter along with the reports is mandatory) You may however, undergo this medical check-up at any center of your choice on direct payment
basis to the maximum reimbursable limit of Rs. 6800 and upload the check-up reports (along with a copy of this letter) to your Health Locker by following the
below simple steps,

For Mumbai location hard copies of the reports along with the PME letter to be sent to: Group Medical Advisor's office at Reliance Industries Limited,

For RCP/ Navi Mumbai/Thane Location hard copies of the reports along with the PME letter to be sent to: Medical and Occupational health Centre, Near

GUIDELINES FOR PERIODICAL MEDICAL EXAMINATION & REIMBURSEMENT
If you undergo this check-up at non-panel center, you will be reimbursed the cost of this medical check-up on submission of a receipt, subject to a maximum of
Rs.6800
To avail this:
1. Upload the check-up reports to the HealthLocker,
2. Login to JioHealthHub --> Select PME from Exclusively For You section --> Upload PME Reports
3. The option for reimbursement will appear in ESS 48 hours after our doctors enter the reports in the medical portal
4. The PME reimbursement option will be available only Once in the stipulated time period
5. Please enter the amount against expenses, upload the scanned copy of bills in a single row only and generate the claim
6. Note: Travel Expenses are not covered
7. Once the reimbursement claim has been generated the PME reimbursement option will be withdrawn form ESS/R-Connect/PeopleFirst
Path for reimbursement: Payroll, Reimbursements and Compensation ->Reimbursement claims-> Create Claim Entry -> Periodical Medical (Select from
Dropdown menu)
INSTRUCTIONS:
1. Minimum of 12 Hrs. fasting is essential prior to the check up
2. May drink water during fasting. Refrain from alcohol at least 12 Hrs. prior to the check up
3. Consult your physician before consuming any medicines on the day of the PME
4. Please carry your regular daily medicines if any along with you
