grandchild, residence of Barangay Linawan, Anda, Bohol, to
request and process the acquisition of the following civil registry
document(s) from the Philippine Statistics Authority (PSA) on my
behalf:

Birth Certificate

Death Certificate

Marriage Certificate

Certificate of No Marriage (CENOMAR)

This authorization covers documents pertaining to Mr./Ms. Requiz,


Ursulina, who is my
brother/sister/child/spouse/father/mother/grandchild/nephew/niece/
uncle/aunt/grandmother/grandfather/myself.

I further authorize Ms. Castigon to receive the requested documents


from the PSA's logistics or delivery courier in my behalf.

Attached is a photocopy of my valid government-issued ID for


verification and validation purposes.
