I. Executive Summary
The Nominee is a grade 6 Teacher in Cutar Elementary School, Aritao,
Nueva Vizcaya. She finished the degree Bachelor in Elementary
Education, major in General Education at Saint Mary’s University in the
year 2016 and is currently on finishing her Action Research for the degree
of Master in Education, major in Educational Leadership at Universidad de
Dagupan, Dagupan City.
The nominee was one of the finalists in the Mathematics Photo Contest
2023 conducted by SEAMEO regional Centre for QITEP in Mathematics.
The said photo was one of the 18 outstanding photos that have been
selected to grace the walls of the International Seminar commemorating
SEQIM’s 14th Anniversary. The exhibition took place at esteemed Ministry
of Education Building in Jakarta, Indonesia.
Extending her community work, the candidate created a collaboration
program with the Local Government Unit (LGU) of Aritao for the resource
creation, production, and distribution of a coffee table book promoting
Aritao tourism and culture. Once completed, this book will be a one-of-a-
kind reading material for Aritao students and the municipality's official
reference.
Significant Accomplishment/s within the Last Three Years
A. Service as a Professional Teacher
As the School English Coordinator, she initiated Project OPERA for
ALL3 (Offering Practical Educational Reading Activities for the Advance,
Lost, Least and Last learners). This project aimed to enhance the reading
ability of the learners, develop among learners the importance of reading,
to help the learners increase their academic performance and most
importantly this project aimed to improve the comprehension level among
learners.
She also initiated PROJECT RICH (Remedial Instruction during Summer
Class for Children’s Hope). This project was conducted during the End-of-
School-Year (EOSY) classes. This aimed to improve the reading fluency of
the learners, to reduce the number of non-readers and to develop the

reading comprehension skills of learners.
She is also the School Paper Adviser of the School: THE BREEZE:
She trained pupils to join the District Schools Press Conference and were
able to bagged Champion and 2nd Runner-up in the News Writing Category
(English).
She initiated the PROJECT BAKOD (Building an Atmosphere for Kids
Outdoors Deck) a partnership project with the stakeholders and alumni.
The project aimed to provide a child-friendly school by ensuring the safety
and security of learners during outdoor activities by constructing a school
fence for a child-friendly environment.

B. Values: As a Responsible Citizen
She served as one of the member during the 2021 National Election.
She served as one of the board of judges during the 2022 Ms. Gay
Pangirayaw Festival wherein it is a part of her extension services.
She also joined in the Pulot Basura Program of Barangay Cutar.
The nominee also served as one of the board of judges in the Search for
Mr. and MS. SWMP of Calitlitan Elementary School.
C. Impact on Community: As an Agent of Positive Change and
Influence
The participation of the nominee in the Mathematics Photo Contest 2023
conducted by SEAMEO regional Centre for QITEP in Mathematics brought
honor and pride to the whole municipality of Aritao.
IV. Other Information ( Major awards, Citation Received)
Finalists in the Mathematics Photo Contest 2023 conducted by SEAMEO
regional Centre for QITEP in Mathematics. The said photo was one of the
18 outstanding photos that have been selected to grace the walls of the
International Seminar commemorating SEQIM’s 14 th Anniversary. The
exhibition took place at esteemed Ministry of Education Building in
Jakarta, Indonesia.
Further, she was commended through a resolution by the Local
Government Unit of Aritao being hailed as the 4 th Runner-up in the 2023
International Mathematics Photography Contest which gives honor and
pride to the Municipality.
2nd Runner-up during the 2023 District Instructional Materials Showdown
last July 21, 2023.
The nominee was also awarded as Gurong Maaasahan for the month of
May 2022 in Aritao 1 District.
