3. APPLICATION

GENERAL APPLICATION OF IFT

Interferential Therapy (Electrotherapy amplitude modulated medium

frequency alternating currents -AMF), combines two effects.

i. Characteristics of an alternating current - not painful

ii. Relatively high carrier frequency - current penetrates deeper into the
tissue. The IFT has got the following treatment features;

1. Two types of frequencies - 4000Hz & 2000Hz

* 4000Hz primarily for analgesic effects and stimulation of the

circulation.

* 2000Hz primarily for Muscle strengthening (Electro

gymnastics).

2. Two electrode placement (2EL - two pole method)

* In this mode, the AMF is generated inside the equipment and

supplied to the patient through two electrodes. This method
makes it very easy to localize the area of stimulation because
the AMF is present in the entire region between the two
electrodes. It is particularly suitable for the treatment of
trigger - points, muscle and skin zones.

TECHNOMED ELECTRONICS 7
VECTROSTIM

3. Four electrode placement (4EL - four pole method)

* The maximum AMF (Amplitude modulated medium

frequency) is obtained in the intersection of the two applied
alternating currents (point of superimposition). Therefore, the
electrodes should be applied accurately. The balance control
enables minor adjustments of the area of effective
stimulation. The four pole method enables selective
stimulation of deeper - lying tissue and is highly suitable of
shoulder, knee, neck and lower back.

4. Four pole method using automatic vector scan (4EL 45 or EL 90)

* In this technique, the intensity of one of the alternating

currents is varied. As a result, a point of super imposition
rotates within the area of intersection enlarging the area of
effective treatment. This treatment is suitable when the point
to be treated cannot be exactly localized.

5. BASE & SPECTRUM variations

* Low frequency (AMF signal) variation can be done through

SPECTRUM & BASE POTENTIOMETERS.

* The adjustable spectrum avoids the adaptation of the tissues to

a particular frequency.

* Rectangular spectrum is the mild one. For chronic disorders

this spectrum is preferable.

* The trapezoidal spectrum is milder than the previous. For sub

acute disorders this is preferable.

TECHNOMED ELECTRONICS 8
VECTROSTIM

* The triangular (linear) spectrum is the mildest. It is used

especially for painful and acute disorders.

GENERAL APPLICATION OF MST

In general LOW FREQUENCY therapy is useful in stimulating the

nerves a bioelectrical current flow in our heart, brain, nerves, and other
tissues, controls a variety of functions of the body and maintains the vital
activities.

If this bioelectric current does not flow smoothly, a balance is lost

and the symptoms such as stiff shoulder and neuralgia may appear. When a
low frequency electric current of the same quality as the electric current
which flows in the human body is given into body by a low - frequency
medical device, the nerves are stimulated, the muscular movements are
arisen, the blood flow is promoted and the symptoms such as stiffness,
pain, paralysis and fatigue can be solved.

1. Galvanic Current

A galvanic current in electrical terms consists of an unmodulated D.C.

This is mainly used to influence the pain threshold and produce hyperemia,
iontophoresis (skin perception) and analgesia. With the simultaneous
application of certain medicines will facilitate the better penetration of the
medicine (known as iontophoresia).

2. Interrupted galvanic

Impulses with a long duration are classified under this category.

They are used for the stimulation of nondenervated muscles.

TECHNOMED ELECTRONICS 9
VECTROSTIM

3. Faradic type

A faradic type current is a short duration current (0.7 m sec) pulse

width and a frequency of 50 - 100Hz. This can be used for Analgesia,
hyperemia and muscle relaxation.

4. Surged faradic type

If the intensity of the successive impulses increases gradually

reaching a peak value and decreases in the same manner, then the type of
current is said to be "surged". Muscular contraction and relaxation occurs
during this surging.

5. Triangular pulsed current

Applications include making I/t curves and treatment of denervated

muscle tissue. These types of waveforms are not painful because of
absence of the sharper edges as existing in the square type of waveforms.

6. TENS (Transcutaneous Electrical nerve stimulation)

TENS uses short duration pulses to stimulate sensory nerves rather

than motor nerves. It encourages the production of ENDORPHIN.

7. Diadynamic Treatment

The Diadynamic currents are able to produce analgesia and

hyperemia.

TECHNOMED ELECTRONICS 10
VECTROSTIM

* D.F.

This is a 50Hz full wave rectified current. Mainly used in the

treatment of peripheral circulatory disorders. At low intensities prickling
sensation will be experienced which will recede when the current strength
is allowed to remain constant. This is due to the elevation of stimulation
threshold.

* M.F.
[

This is a 50Hz half wave rectified current. This seldom used alone in
treatment, as it is present in the C.P. & L. P. forms.

* C.P.

This is full wave rectified 50Hz alternating with half wave rectified
in two second cycles. This current has a strong anagesic effect causing
muscular relaxation and improving the local blood supply. It is also
suitable for treatment of sprains, contusions, neuralgia, facial paresis and
sciatica.

TECHNOMED ELECTRONICS 11
VECTROSTIM

* L.P.

This is based on a 50Hz continuous half wave with steadily

increasing and decreasing amplitude of the other half wave. The
current has pronounced anagesic effect with applications in the
treatment of myalgia. The sensations experienced by the patient are
similar to those of C.P. described previously.

13. BALANCE CONTROL

If there is a difference in the currents of both channels, this can be

equalized by adjusting the intensity control (13) provided for this purpose.
Usually these differences are caused due to the variation in resistance in
the body where the two current are passing.

14. INTENSITY CONTROL

The increase of current (dosage) can be done by rotating this control

in clockwise direction, cautiously and gently, until the patient starts feeling
the current. It can be increased until the patient can tolerate. The current in
channel 1 and channel 2 are independently measured.

TECHNOMED ELECTRONICS 20
VECTROSTIM

SAFETY: Once the treatment is over this control should be brought to

the initial position by rotating it in anticlockwise direction.
Incase if this control is not in minimum position, the unit will
not deliver the current to the patient as a precaution, until you
brought it to minimum position. All through you will hear a
warning signal and display of the message "INTENSITY
KNOB NOT AT ZERO" on LCD display.

15. BASE CONTROL

This knob sets the Base frequency of the channel

16. SPECTRUM CONTROL

This knob sets the Spectrum frequency of the channel 1.

17. CHANNEL 1 OUTPUT TERMINAL

* This delivers channel 1 current of both IFT & MST.

* During 2 electrode (IFT) Programme this terminal delivers the

mixed composite current of channel 1 and channel 2 which are
mixed inside the unit itself. (Bipolar interface)

18. CHANNEL 2 OUTPUT TERMINAL

* This delivers the channel 2 current of both IFT & MST.

* This will not deliver any current during 2 electrode mode.

6. OPERATION & PROGRAMMING PROCEDURE

1. Choose which treatment to be given, whether IFT or MST.

2. Decide what type treatment to be given. (For e.g. 4 kHz or 2 kHz,

2EL or 4EL or 4EL90, spectrum square or triangle).

3. The lists of all the combinations with the corresponding program Nos.
are given in section 8 & 9. Pick out the desired program number.

4. Switch on the equipment. Confirm that whether the POWER cord is

in the POWER socket (l). Note: The numbers within the brackets are
referred to the schematics Fig. 1 & Fig. 2.

5. Moisten the electrodes and attach them with the cables provided.

6. Fix the electrodes on the body of the patient at the position in such
way that the output current crosses at the area to be treated.

7. When using bipolar therapy (2 Electrode), use only Channel 1 socket

(17). In the Bipolar technique the currents of two channels are the
superimposed inside machine and delivered through Channel 1 output
socket.

8. Follow the systematic programming approach given in Fig. 3 for IFT

and Fig. 4 for MST. Make sure that the intensity control is increased
gradually to the level tolerated by the patient.
