Section 2: Assumption of Risk, Release, and Waiver of Liability
In consideration of the minor child being permitted to participate in any and all
activities organized, operated, or sponsored by the above-mentioned league or
organization (hereinafter referred to as the “League”), the undersigned parent or
legal guardian acknowledges and agrees to the following terms on behalf of
themselves and the minor child:
1. Assumption of Risk: I am fully aware that participation in sports involves
inherent risks, including but not limited to bodily injury, illness, permanent
disability, or death. I voluntarily assume all such risks, whether foreseen or
unforeseen, related to participation in League activities, including but not
limited to practices, games, travel, or events.
2. Release of Liability: I, on behalf of myself, my child, and our respective
heirs, assigns, personal representatives, and next of kin, do hereby release,
waive, discharge, and hold harmless the League, its directors, officers,
agents, employees, coaches, volunteers, sponsors, affiliates, and any facility
used by the League from any and all liability, claims, demands, causes of
action, or damages arising out of or in connection with participation in League
activities, whether arising from negligence or otherwise.
3. Medical Authorization: I hereby authorize the League and its
representatives to seek emergency medical treatment for my child in the
event that I cannot be reached in a timely manner. I accept full financial
responsibility for any medical treatment deemed necessary.
4. Code of Conduct: I acknowledge that my child is expected to abide by the
rules, regulations, and code of conduct established by the League. I
understand that failure to comply may result in disciplinary action, including
suspension or dismissal from participation.
5. Photographic & Media Consent (Optional – check if agreed):

taken during official activities for promotional, educational, or informational
purposes.
