FIELD REPORT FORMAT
PRELIMINARY PAGES (Roman numbers are used to indicate their page numbers)
Declaration (is a formal announcement about the ownership of the document)
Acknowledgements (acceptance of the truth or recognition of the importance of all people
who helped you by one way or the other to accomplish the job)
Dedication (A dedication is the expression of friendly connection or thanks by the author
(student) towards another person).You can dedicate your job to your parents, spouse etc.
Acronyms (an abbreviation formed from the initial letters of other words and pronounced as
a word (e.g. ASCII, NASA)
Abstract (a summary of the contents of the report. Normally written last but appears in the
beginning pages first)
Table of Contents (usually headed simply Contents and abbreviated informally as TOC, is a
list, usually found on a page before the start of a written work (report), of its chapter or
section titles or brief descriptions with their commencing page numbers).
CHAPTER ONE-INTRODUCTION PART
1.1 Background information of the Organization (this is provided by the organisation’s
leadership/management)
1.2 Vision (it is obtained from the organization you were attached, don’t formulate your own
vision)
1.3 Mission (it is obtained from the organization you were attached, don’t formulate your
own mission statement (s))
1.4 Type of business/activity or services delivered organization (it should be explained to
indicate the beneficiaries or customers/consumers of that particular service or product the
organization is offering)

1.5 Organization Structure (obtained from the organisation; It should be chart showing
reporting system or authority relationship)
CHAPTER TWO-TECHNICAL PART
2.1 Description of duties at the field station (explain all activities you have been doing
during field attachment, this part is well assisted by your log book-daily records. This is the
most important part of your report. It reflects the whole field practice. Avoid exaggeration
during explaining this part)
2.2 Lesson learnt (a well written part 2.1 above will help you to identify the all aspects learnt
from the work place. It reflects the essence of field practical attachment. Failure to explain
this will lead into repetition of the whole exercise
2.3 Problem Identification (a well explained part 2.2 above will help you to identify the
problems/challenges you faced during your field attachment. Challenges can be obtained
from procedures of delivering the services or processes that are involved in production. But
again, challenges can be identified from the policies used by the organization to guide
decision making.
2.4 Discussion (this part reveals your ability to think. It is the area where you can introduce
your own ideas on how the organization/company/firm can improve its operations by
changing its policies, procedures, regulations, organization structure etc. Your discussion
must be well connected with part 2.3 above. Although references can be used right from the
beginning (chapter one) of the report but this part is the most important in showing your
referencing ability. You can use books, articles, journals, speeches, meeting minute’s e.t.c to
support your ideas. It is where most of the theories acquired from the class meetings are well
used. References must be included in this subsection.
2.5 Conclusions and Recommendations (conclusion refers to the summary of the above
content, particularly within the chapter. It summarizes the whole chapter. It an opinion you
have in the whole content. Recommendation refers a suggestion or proposal as to the best
course of action, especially one put forward by an author. As a writer of the report you have
an authority to recommend to the higher authority/authorities of that particular organization
on what is to be done to improve the situation. In this case, recommendations can also be
directed to the university on how it can improve the whole field attachment practice.
LIST OF REFERENCES
Note; this is not a separate chapter; this is a list of all sources used in report writing. This list
includes Books, journals, speeches, meeting, electronics sources e.g. Wikipedia etc.It appears
in the last page(s) of the report.
