INTRODUCTION
As the part of our M.Sc. Nursing programme, our institution KGMU, arranged Community
posting and for that they have selected Sarojini Nagar PHC. I Sidrah Javed Rehman
student of M.Sc. Nursing II year of Community Department. I was by my teacher Clinical
Instructor MS. SARITA SINGH.
OBJECTIVES :
• To observe the staffing pattern of the PHC.
• To know about the services provided by the PHC.
• To get the knowledge about the programme running under the PHC.
• To get the knowledge, how to provide services at PHC level.
• To know about how to provide care to the individual, family and community.
PRIMARY HEALTH CARE
• Primary health care is a new approach to health care which integrates health at
community level. A new approach to health is came into existence in 1978,
following an international conference at Alma Ata (USSR). This is known as
“Primary Health Care”.
• Before Alma Ata, primary health care was regarded as synonymous with “basic
health services” “services provided by generalists”.
The Alma Ata Conference defined primary health care as follows-
“Primary health care is essential health care made universally accessible to individuals and
acceptable to them, through their full participation and at a cost the community and country
can afford”.
CONCEPT OF PRIMARY HEALTH CARE
According to department of health primary health care is characterized by partnership and
empowerment of people that shall permeate as a core strategy in the effective provision of
essential health services that is community based, accessible, acceptable and sustainable at
a cost which is the community and government can afford and strategy to reach the goal of
health for all in 2000.
ELEMENTS OF PRIMARY HEALTH CARE
Although specific services provided will vary in different countries and communities, the Alma
Ata Declaration has outlines 8 essential components of primary health care:
1. Education concerning prevailing health problems and the methods of preventing
and controlling them.
2. Promotion of food supply and proper nutrition.
3. An adequate supply of safe water and basic sanitation.
4. Maternal and child health care, including family planning.
5. Immunization against major infectious disease.
6. Prevention and control of locally endemic diseases
7. Appropriate treatment of common diseases. 8. Provision of essential drugs.

DISTRIBUTION OF PRIMARY HEALTH CARE SERVICE
Education about prevailing health problems and methods of preventing and controlling
them
1. Promotion of food supply and proper nutrition
2. An adequate supply of safe water and basic sanitation
3. Maternal and child health care, including family planning
4. Immunization against infectious diseases
5. Prevention and control of endemic disease
6. Appropriate treatment of common diseases and injuries, and
7. Provision of essential drugs.
8. Improve the health status of the population.
9. Mortality and morbidity rate reduction and increase in expectation of life.
10. Reduced levels of poverty.
PRIMARY HEALTH CENTRE
Primary Health centre are the corner stone of the rural helth services. It acts as a referral
unit for 6 sub-centres and refer out cases to CHCs. It covers a population of 30,000 in plain
area and 20,000 in hilly and tribal area. There are 4-6 beds for patients and some diagnostic
facilities are also available.
“Primary Health Centre is the basic structural and functional unit of the public Health
services in developing countries, to provide accessible, affordable and available primary
health care to people”. Primary health centres and subcentre now referred to as “Health
wellness Centre”.
OBJECTIVES:
• To provide comprehensive primary health care to the community at PHCs.
• To achieve and maintain an acceptable standards of quality of care. • To make the
services more responsive and sensitive to the needs of the community.
FUNCTIONS:
• Medical care
• Treatment and prevention of endemic diseases.
• Maternal and child health care.
• Health education.
• Referral services.
• Basic laboratory tests.
• National health programmes.
• Training of ANM and ASHA.
• Collection and reporting of vital events.
GENERAL DESCRIPTION OF PHC, SAROJINI NAGAR
Location and Landmark:
The Primary Health Centre of Sarojini Nagar is located at about 25 kilometres from the
main city, Lucknow on the way of Kanpur road. Nearby PHC, there is Chaudhary Charan
Singh international airport of Lucknow. A Sainik School before reaching Primary
Health Centre and just beside Community Health Centre is situated.

Governing Body And Affiliation–
P.H.C of Sarojini Nagar is governed and affiliated by government under King George’s
Medical University, Lucknow.
OTHER RELATED INFORMATION REGARDING P.H.C-
Area-The area is covered by the PHC is 12 kilometre* 5 km=60sqkm.It covers large area
and each department of PHC is designed very nicely.
Population-The population covered by PHC at Sarojini Nagar is 93,262.
Number of village-There is about 41 village under PHC at Sarojini nagar.

There are 26 beds in the P.H.C. Sarojini Nagar, which are divided as follows

DESCRIPTION OF P.H.C. PHYSICAL SETUP
• The set up of P.H.C. at Sarojini Nagar is very nice. In the front it has a big gate for the
entry of the people. By entering there is a garden at right side.
• The main office of Director of PHC and two well maintained meeting room is situated
in the left side along with Seminar hall.
• The pathway divided into two ways: one for the main building of PHC and other for the
CHC. The straight pathway leads forward from main entrance of PHC units along with
a DOTS centre.
• While entering into the PHC building first we reached the registration counter combined
with pharmacy. And at the left side of the pharmacy there is a combined injection room
and dressing room and there is dental centre on left side of pharmacy. • The pathway
took us towards clerk Office and homeopathy room.
The area in front of clerk office is utilized as Immunization room.
• At the right side of the main pathway there is General Ward, Store Room, Pathology
and Cold Chain Room.
• Department of ophthalmology is situated at the back side of the cold chain room.
• The straight pathway leads to the Gynaecology OPD, Maternity ward 2nd, nurse’s station
along with that labour room attached.
• Afterwards this pathway takes towards Eye OPD and AYUSH OPD at left side and at
the right side General OPD.

DESCRIPTION OF DEPARTMENTS:
1 MEETING HALL: Various training programmes are used to held in meeting hall
like pulse polio, Indradhanush Programme ,Ayushman Programme, etc.
2 DRESSING ROOM: In dressing room sterile pads or compress is applied to
wound to promote healing.
3 PHARMACY: Free of cost medicines are supplied to patients as well as
community people.
4 INJECTION ROOM: Immunization of children as well as antenatal mothers is
performed.
5 PATHOLOGY; Blood samples, urine samples are collected and investigated.
6 GYNAE OPD: Dr. Sunita Johri is consultant of gynae department. Mothers come
for antenatal visit, antenatal checkup as well as postnatal examination are carried
out. In some cases pap smear is also taken for diagnostic purpose.
Timing: 9:00am -4:00pm.
7 MATERNITY WARD : Mothers stay for few days after delivery of baby.
Postnatal care is provided by health care workers, they used to teach her about
breast feeding techniques, handling of baby and minor symptoms of newborn.
8 LABOUR ROOM: It is adequately equipped and staffed according to need,
delivery is being conducted here throughout the year. Mainly normal vaginal
delivery is preferred, if in case any emergency or complication arises referred to
higher institutes having more amenities.
9 COLD CHAIN ROOM: It is a temperature control supplied chain. Here vaccines
are stored at their recommended temperatures and it is transported via vaccine
carrier and day carrier from PHC to village areas at the time of immunization
programme.
10 EYE OPD: Mainly community people who are having eye problems visit eye OPD.
Old age people diagnosed as a case of glaucoma and cataract are given therapeutic
treatment.
11 EYE OT: For cataract surgical intervention has been performed.
12 GENERAL OPD: Patient who is suffering from pyrexia, diarrhoea, vomiting,
urinary retention, cold and cough, minor injuries and other health related problem
visit in general OPD.
13 AYUSH: Primary health care deals with basic medicines AYUSH is one of them,
where patients are treated via using Indian medicine system.
14 DOTS CLINIC: DOTS or directly observed treatment short course is the
internationally recommended strategy for TB control that has been recognised as a
highly efficient and cost effective strategy. Free of cost ATT drugs and a help of Rs
500 per month is given to the community peoples suffering from TB.

ACTIVITY PERFORMED :
During our posting in PHC sarojini nagar we performed nukkad natak on menstrual hygienic with
the help of B.Sc nursing 4th year students.
Through this nukkad natak we provide all the hygienic activities to do during this menstrual cycle
to the adolescent girls and women.
We distribute pads packet to the women and girls with the support of nine foundation.
