Step Control Measures/ Responsible

1 Work start-up & obtaining  Unauthorized work  Obtain a hot work permit before starting the job PTW Applicant.

 Invalid permit  Supervisor must ensure that all work activities are ‘PTW’
carried out by competent personnel (Permit to work)
 Lack of competency  Conduct Toolbox meeting; review the Job safety
analysis to all craftsmen involved in the work
 Lack or poor communication activities.
 Responsible person will conduct inspection,
 Haste or shortcut monitor the work activities and have to ensure
safety standards and procedures are strictly
adhered to.

2 Welding
 Before starting welding make sure the vessel is
 Fire Hazard clearly flushed and drained of all types of  Process
 Burns. combustible materials Operator
 Electrocution/ Spatter burns  Site
 Provide a fire blanket on site
 Damage cable Supervisor
 Make sure fire extinguishers are available on site
 Hot tabs end pieces of welding  Site safety
 Wear proper PPE for welding ie., leather jackets, Officer
rods.
 Sparking Gloves, welding goggles/welding mask,
 Trained Fire Watch.
 Gas Monitor correctly sited.
 Make sure the vessel is wet and checked with
portable gas detector any presence of flammable
gases traped.
 The MEG contained in the vessel should be
flushed and drained.
