certain Garbejud Q. Quereuan, 19 years old, single, jobless and presently resides at Prk. Sora,
Quirante II, Tagum City was caught on the act of stealing one (1) piece Giordano Polo shirt with
Selling Retail Prize (SRP) of Seven Hundred Ten pesos (P710.00) intercepted by the
undersigned thru the tip information given by the Watcher NUP and resulted to the successful
execution of citizens arrest to the suspect.

After initial inquiry, it was found out that the above-mentioned suspect concealed the
said polo shirt underneath his jeans waistline while at Men’s Wear Section, Department Store
area, where he was spotted by the Watcher NUP, after which the latter proceeded at the
customer’s exit and did not pay the item at the cashier’s counter. The suspect revealed that he
was just force to commit shoplifting due to financial problem.

Aforementioned suspect including the intercepted evidence were immediately endorsed


at the PNP Station for custody and appropriate action. Thus, the undersigned recommends for
immediate filing of appropriate charges against the suspect.

I-Authority:
1. As per directive by the Operation Manager of RD Security Agency to conduct
investigation of the incident.

III-Facts of the case:


5. On or about 1230H 07 January 2017 four (4) suspects pretending as bank client, one
of them at gun point disarmed SG. Puerte K. Lhoya while the three (3) other suspects ordered
to drop everyone inside the bank and declared holdup, attached copy of logbook record marked
as Annex “A” of this report. After one (1) minute the suspects went out from the bank carted
cash amount ofP14,300,025.00 on board their two (2) getaway motorcycle bound to J.P. Laurel
Avenue going to unknown direction. Hereto attached an excerpt copy of police blotter marked
as Annex “B” hereof.
6. Immediately after the incident, the said security guard dialed 911 for police
assistance. The police arrived and conduct investigation while some police mobile patrol trying
to pursue the suspects.

IV- Discussion:
7. The bank has two (2) security guards at a time, but happens to be that during hold-up
incident it was only one (1) security at the entrance while the other one took his lunch outside.
8. It was in customary system of the security guards that during lunch time, personal
necessity and other natures call, only one (1) guard left at the bank manning the entrance for
security and safety.
9. On the record of RD Security Agency office found out that last December 1, 2016 the
operation manager Mario P. Lua submitted security survey to CBC Bank management received
by Bank Account Officer Gina Aguanta recommending for additional one (1) security guard in
order to maintain two guard system even during lunch time for effective security measures. The
proof of which attached is a Security Survey report marked as Annex-“C”.
V-Findings/Conclusion:
10. The bank was clearly outnumbered of security guard during hold-up incident, as one
security guard on duty at the time took lunch break outside.
11. The incident was clearly committed with force majeaure wherein under this
circumstance the guard on duty is helpless to prevent the incident. Thus the guard cannot be
held liable.

VI- Recommendation:
12. Additional one (1) security guard is necessary for efficient implementation of security
and safety during banking hour operations. With this, the bank shall not be left unmanned and
maintain two security guards even during lunch time.

A. INTRODUCTION:
Authority:
1. Pursuant to the instruction from SCC HR Manager to conduct Security Survey
to determine the vulnerabilities, threat and hazards that may result to potential risk in the
business assets whether lives and property inside the Sanitary Chemicals Corporation
(SCC) at Binugao, Toril, Davao City.
2. Standard Operating Procedure. (SOP)
Date and Hour Survey was made:
Security Survey was conducted last March 17, 2022 from 4:05 PM to 07:12 PM.
Surveying Personnel:
The security survey was conducted by Cesar M. Caña, officer of AZ SECURITY
AGENCY and Ms. Jane P. Santos, HR Manager from Manila main office and Ms. Henny
Abu , OIC Davao Office both of Sanitary Chemicals Corporation.

B. NAME AND MISSION OF SURVEYED UNIT

Description & Location:

Name of property surveyed is Sanitary Chemicals Corporation (SCC), located at


Binugao Toril, Davao City with one (1) gate utilized as entrance/exit in the SCC
compound. Structure inside is the separate Office building where the OIC, Accounting
and Office Staff located wherein customers and visitors will be entertained. The other one
is the Production Building where the laboratory, mixing area, chemicals located and
being used by chemist on their mixing activity for the production of the company. The
whole compound has a lot area of more or less 700 square meters.

Mission of Surveyed Unit

 The Sanitary Chemicals Corporation is engage in manufacturing and whole sale of


Cleaning, Disinfectant & Sanitizers products usually utilize by restaurants, hotels,
and residences for dish washing and with expertise in Cleaning In Place (CIP),
Environmental Cleaning & COP, Hand Hygiene, Disinfectant & Sanitizers,
Process Improvement, Housekeeping & Laundry, Quick Service Restaurant
(QSR), Dispenser & Equipment and others more.

 This security survey is made to assess the present security set-up and determine
security risk and recommend preventive measures for target hardening against
Theft whether internal and external theft, Robbery, Terrorist attack, Bomb threat
and Explosion, Fires, Kidnapping, and any other unlawful acts and hazards. This is
to further determine the weaknesses of the present security system/barriers and
line of defense in minimizing or possibly preventing the aforementioned threats.
 To determine and identify the defects of the existing security and safety protection
system on the area and to recommend possible solution to enhance its physical
security and safety measures for implementation thereat.

C. PHYSICAL SECURITY:

1. Perimeter Barriers
a. Is used to define the boundary of the property to be secured,
b. Create physical and psychological deterrence to any unauthorized entry,
c. Delay intrusion, thus facilitating apprehension of intruders,
d. Assist in a more efficient and economical employment of guards, hence
reducing the need for employing additional security personnel.
e. Facilitate and improve the control of pedestrian and vehicular traffic.

1.1 Situation:
The Sanitary Chemicals Corporation compound surrounded with standard
solid perimeter fence with barbed wire above pointing upward served as top
guard. However, on the side fence near on the newly constructed propose
laboratory is nearby neighboring residences which could possibly an access the
pilfered stocks from inside by throwing outside that cannot be seen by the guard
stationed in the guardhouse which visibility blockade by wall of the office.

Recommendation:
The prescribed top guard should be 45-degree angle facing upward
outward for effective prevention from unauthorized person or lawless elements
passing through above the perimeter fence. Though there is no impenetrable
barrier by principle, but top guard provide delay to intruders and facilitate
apprehension.

1.2 Situation
The Door of the Production Warehouse was damaged cannot be used to
close the door of the plant to become restricted area.

Recommendation:
To install door to always close the production warehouse specially after
working hour and that should be equipped with lock which then be constantly
checked by the guard on duty. The close and lock door signifies restricted area,
more so the laboratory is situated inside and that unauthorized person is not
permitted to enter unless authorized by the management.

2. Guard Force and Guard System:

Situation:
Present set-up:
There are two (2) guards manning 24 hours rendering 12 hours duty in two
shifting, one guard for dayshift and one guard in night shift.
1. One (1) Day-Shift Guard – detailed as Gate/Entrance guard from 8:00AM to 8:00PM
with the following duties and responsibilities:

1.1 Assumed duty at least 15 minutes before you tour of duty at 8:00AM and ensure
the security and safety of Sanitary Chemicals Corporation,

1.2 Attendance of employees should be monitored in the Logbook record;

1.3 All trucks/vehicle of the company checkout for deliveries outside is subject for
inspection of the guard on duty;

1.4 All Bags/Anything carried by employees should be subject for inspection before
going out from the compound;

1.5 Monitor and record in the logbook the over and lengthy break time that excess
the required time duration given by the management;

1.6 Monitor and check the employee’s duty time to ensure they are working by
conducting roving inside the Production Warehouse at 8:15AM, 10:15AM,
1:00PM and 3:15PM. Any unusual observation of employees during working
hour should be recorded in the logbook and reported to the management.

1.7 Logbook all visitors entered the SCC compound to the Visitors Logbook.

1.8 Disallow deadly weapon to any person entering the compound;

1.9 All doors of SCC Office and Production Warehouse must be checked by the
guard at 6:00PM after check out all employees and continue to monitor by
conducting roving of the entire SCC compound.;

1.10 No employees allowed to go out during office hour except during their
break time and lunch time. However, during office hour in case of necessity or
important transaction they should present GATE PASS approved and duly signed
by OIC or authorize representative;

1.11 Submit weekly report of the Logbook to SCC OIC, In-Charge.

1.12 Any unusual observation or incident should be recorded in the logbook


and submit incident report to the SCC management. The Security Agency Office
should also be informed with copy furnish of the report.

2. Night-Shift Guard – Detailed as monitoring guard during night time from 8:00PM to
8:00AM focus on the gate/entrance and monitor the perimeter fence as well as the
entire SCC compound. The responsibility of Night-Shift Guard is generally the same
with the abovementioned task of the Day-shift Guard. Everything should be recorded
in the logbook and conduct roving to constantly check all doors of SCC in Production
Warehouse including the office..

Recommendation

1. Ensure that the guards assigned are constantly alert while on duty and
doing well their duties and responsibilities.
2. Any infractions/violations of Security Guards to said policy should be
reported to AZ Agency Office from which the agency can give
appropriate disciplinary action to the guard concern.
3. Increase the contract rate of the Security Guards into a minimum wage
rate to boost morale of the guards assigned thereat and both parties are
free and harmless to labor suit or even during DOLE personnel routine
inspection;
4. The biometric should be transferred at the guard house near the main
gate that should be monitored by the guard on duty;
5. Install Buzzer Bell at the entrance connected to the production
warehouse which should be controlled by the security guard on duty.
A buzzer will be switch on at 10:00AM as employees’ morning break
time and 10:15AM to signal them to go back to their respective
workplace. Again buzzer bell switch on at 12:00 noon for their lunch
break then again at 1:00PM to start their afternoon duty. Then again
buzzer will be on at 3:00AM as their break time in the afternoon duty,
and buzzer again at 3:15PM then go back to their work.
6. Ensure that the vehicle gate and the door used as ingress / egress in SCC
compound should be closed and lock all the time. Will be open only to
authorize person like employees and visitors after verification with legitimate
purpose. Never open the door to stranger without prior notice or appointment
schedule and consultation from the OIC or SCC authorize representative.
Except those /customer of SCC and duly verified to the OIC by the guard on
duty.
7. The Wall Clock at the guardhouse area needs new battery to become
serviceable and should be set synchronize to the SCC Management time.

3. Personnel Identification and Movement Control:


Situation:
Notice that the company has already existing policy in terms of personnel
identification and movement control. This should not be given less priority since inside
theft/fraud could be easily committed by employees if the company has no proper
movement control system.

Recommendation:
1. Always implement wearing Identification (ID) Card as manifestation of
employee’s loyalty to the company and wear uniform while in the work
place. They should not be allowed to enter the premises without access pass
or approval from the management whenever they are not wearing uniform or
forget their ID Card upon implementation. Prohibit strictly those who are not
wearing Personal Protective Equipment (PPE) inside the production specially
those assigned in mixing area.
2. Smoking cigarette inside the premises should be prohibited to prevent fire
hazards and the company must maintain the policy that any fix assets of the
company shall not be allowed to checkout from the premises without “Gate
Pass” duly approved and signed by the authorized representative. In the case
of Stocks Delivery, the bearer must present Delivery Receipt (DR)
containing the number of stocks/items and that should be check by the guard
on duty. Strict implementation of body frisking and inspection of bags of
personnel upon checking-out the premises must be done.

4. Protective lighting:

Situations:
Some light still not functioning that results lack of illumination during
night time inside the compound particularly in front the door of newly constructed
propose laboratory. NO emergency light and generator set in place in case of
power interruption that probably disrupt business operations and decrease its
workers production output.

Recommendation
1. Install one light for night time outside the door of newly constructed
building to illuminate the bodega and the fence nearby residences. See
to it that all electric lights are constantly check by authorized
electrician to preempt possible short circuit as number one causes of
fire incident. In case of busted light should be immediately replaced.

2. Install generator set in the area to supply electricity to office,


laboratory and production area purposely to continue its business even
in time of power interruption.

5. Alarm-Fire Alarm and Communication System:


Situation:
The SCC compound has no fire alarm system and fire hydrant and even
smoke detector as necessary requirement by government agency to prevent fire
incident. The company has its owned landline telephone and all key personnel
have its owned mobile cellular phone that can be used immediately whenever
there are necessity and situation demands.

Recommendation:
1) Highly recommend to install Smoke Detector Alarm System inside the
Office Staff, laboratory and in the Production Warehouse to have an
early detection while in the early stage of fire.
2) Also recommend additional two (2) fire extinguisher inside the
Production Warehouse and one(1) in the office staff, one (1) in the
laboratory and one (1) in the guardhouse area.
3) Installation of Fire hydrant is also recommended.

6. CCTV Camera:

Situation
The area is equipped with CCTV Camera but some areas needs
monitoring specially to determined vulnerable to access.
Recommendation:
1. Installation of one CCTV Camera outside the newly propose laboratory
fronting the side of the fence nearby residences to monitor the possibility of inside
job that stocks can be thrown outside passing above the fence in front side of
newly constructed laboratory, and that camera will also monitor the in/out of any
person/employee at the laboratory entrance door.
2. Repositioning of one CCTV Camera from the outside of old laboratory
area facing to the perimeter view fronting the gate to catch possible intruder
passing through the fence.
3. Install one (1) CCTV Camera from the office building facing towards
the guardhouse, viewing entrance door and main gate including the guard house..

D. PERSONNEL SECURITY

1. Data Concerning Key Personnel;


Recommended Guidelines/SOP:
a. The data of every personnel is very important especially those positions with high
responsibility. They should be constantly check as to their loyalty, dedication of doing
their job and personal necessity. Since the personnel with big responsibility has given full
trust and confidence by the management.
b. Establish and maintain key control. The key should not be left by any body to avoid
being duplicated by anyone else. It should be control by one trusted person.
2. Clearance Program:
Recommended Guidelines/SOP:

a. Updated clearances of employees yearly, like Barangay clearance, police clearance,


NBI clearance, Drug Test and Neuro test. Including sketch map of present address and
provincial address and submission the names of their parents, brothers and sisters with
their respective address and contact numbers.
b. All on leave, absent, day-off and suspended employees should not be allowed to enter
the premises unless with proper clearance/approval thru access pass duly signed by the
authorized representative by the management.

c. Whenever the door is already close, it should not be open even by the designated
personnel handling the key unless with proper clearance or approval from the
management/OIC.

d. During working hour, the employee shall not be permitted to go out aside from break
time. But going out without legitimate purpose must present Clearance/Approval from
the management.

e. Those leave of absent due to illness or being hospitalized or undergoing medical

permitted to resume on his/her work.

f. Outgoing delivery, the driver must be given Trip Ticket clearance before granted to
checkout or deliver items/stocks outside aside from the Delivery Receipts.

3. Security Education Program:

Recommended Guidelines/SOP:

a. The personnel/employees must undergo seminar/orientation with regards to security


and safety program of the company.

b. The Security and Safety responsibility of the entire SCC premises and its assets is not
only the concerned by security guard on duty but it is the concerned of everybody in the
workplace. As such Security Awareness among personnel is significant for effective
cooperation in Security & Safety aspect to complement guard’s primary duties. As not all
corners of SCC compound can be monitored by the guard.

E. OTHER SITUATIONS:
Situations:
1. The scrap, empty galloons of chemicals and waste materials were scattered
near the bodega fronting the door of newly constructed building that will create fire
hazards during high temperature of sunlight.

Recommendation:
Such scrap materials, empty galloons of chemicals and waste materials should be
properly disposed to prevent fire incident.
