requesting and claiming an official copy of my Marriage Certificate from the Philippine
Statistics Authority (PSA).

Below are the relevant details:

 Name of Wife: Dareen Jean Mapili Dumdum
 Date of Marriage: January 25, 2025
 Place of Marriage: Peñaplata, Samal City, Davao Del Norte

The reason I am unable to personally request the document is not available due to personal
matters.

Attached are the following documents for your reference:

1. Photocopy of my valid government-issued ID with signature


2. Photocopy of the valid ID of my authorized representative with signature
3. Signed copy of this authorization letter
