AFFIDAVIT OF LOSS

I, MARILYN CABAL ASUNCION, Filipino, of legal age, married


and with office address at Zone 1, Iba, Zambales, being first duly
sworn in accordance with law, depose and state that:

1. I am the registered owner and proprietor of Iba Diagnostic


Center located at Brgy. Palanginan, Iba, Zambales;

2. As a registered business, I have a Business Name Registration


form from the Department of Trade and Industry;

3. However, on 26 June 2013, I noticed that my Business Name


Registration Certificate No. 01307288 was missing;

4. I searched high and low for the Business Name Registration


Certificate but still could not locate the same;

5. I hereby expressly state that the aforementioned Business


Name Registration Certificate can now be considered, for all
legal intents and purposes, lost beyond recovery; and

6. I am executing this affidavit to attest to the truth and veracity


of the foregoing facts.
