hereby directed to submit your counter-affidavit and other supporting

the City Prosecutor, 2nd floor, New Hall of Justice Bldg., Caloocan City.
Attached herewith as “Annex A” is a copy of the complaint-affidavit and
other evidence submitted by the complainant.
You are hereby WARNED that failure on your part to comply with
this subpoena shall be considered as a waiver of your right to present
your defense and the case shall be considered submitted for resolution

COMPLAINT – AFFIDAVIT
I, KATIE P, of legal age, Filipino, common-law partner of herein
respondent, and a resident of Barangay Masinloc, Caloocan City, after
having been duly sworn in accordance with law, hereby depose and
state:
1. I am and the same person who is the complainant in the instant
case;
2. I know the person of PAPA P, being my common law partner for
about ten (10) years now, and is currently residing with me at
Barangay Masinloc, Caloocan City;
3. That in the later years of our relationship, his behavior towards
me has become increasingly abusive;
4. That he frequently belittles me, and calling my name like "stupid,"
"worthless," and "crazy;"
5. That he often does this in front of my family and friends, causing
me deep shame, emotional distress, and humiliation;
6. That he constantly criticizes my appearance, my cooking, and
my personal opinions;
7. That he is extremely jealous and possessive, and he constantly
accuses me of being unfaithful without any basis or evidence.
8. That such actions abovementioned of herein respondent have
caused mental or emotional anguish which affects my mental
and psychological functions;
9. That the acts aforementioned are violative of Section 5(i) of
Republic Act No. 9262 or Anti-Violence Against Women and
Their Children Act of 2004 which provides that:
“Causing mental or emotional anguish, public ridicule or
humiliation to the woman or her child, including, but not
limited to, repeated verbal and emotional abuse, and denial
of financial support or custody of minor children of access
to the woman's child/children.”
PRAYER
WHEREFORE, premises considered, it is most respectfully
prayed of this Honorable Court that after due notice and hearing,
judgment be rendered in favor of Complainant:
1. For the issuance of temporary protection order from respondent;
2. For the respondent to move to another place and not to reside
anymore with herein petitioner;
3. Other reliefs just and equitable under the premises are likewise
prayed for.
