NOTICE TO OUR GUEST(S):

1. A personal electronic safety deposit box is installed in your room closet. The MANAGEMENT shall not be
responsible for any loss of money, jewelry, and other valuables kept and left inside the Resort room.
2. DAILY RATE is subject to 10% service charge and applicable government taxes. CHECK in time 2:00pm
and check out time is 12:00nn.
3. A replacement fee of PHP 1,500.00 will be charged for every lost key.
4. For security reasons and as courtesy to other guests, PARTIES ARE NOT ALLOWED IN THE HOTEL
ROOMS. Applicable corkage fees shall be charged from the Front desk should guest bring up to the room
food and beverages items in excess of the normal consumption of the maximum number of occupants in the
room. The maximum number of occupants in the rooms as follows: (2) TWO persons for a DE LUXE ROOM,
(4) FOUR persons for a LUXURY ROOM and (6) SIX persons for TWO LUXURY ROOMS. Visitors are allowed
until 10pm only or corresponding charges will be automatically made for every excess person.
Gambling inside the resort rooms are strictly prohibited. Unauthorized pictorials and/or video
shoots of the resort's room s and guest public areas are strictly prohibited. Prior written
permission from management is required. Any infractions commited shall be subject to
corresponding fees.
5. TO THE ABOVE RESORT: I/We acknowledge that I /we am / are liable (jointly and severally) for the
payment of all costs and charges payable or incurred in connection with the services provided under the
registration and I/ we hereby authorize CMC HOTEL to automatically debit any unpaid charges such as
DAMAGES in the room arising from MISUSE and/ or VANDALISM, including but not limited to MISSING
RESORT ITEMS/ PROPERTY from my / our personal account/ credit card. Furthermore, I/ we hereby to pay
a CLEANING FEE equivalent to a one (1) night room charge should extensive cleaning Be required to
return to the room to its good condition after any misuse.

6. The hotel may use the information stated in this registration formfor marketing activities it may embark
on.
