I. RATIONALE

A PWD Week Program is a vital initiative to advance the rights and well-being of
persons with disabilities while fostering a more inclusive and equitable society. By
raising awareness, empowering individuals, celebrating achievements, and advocating
for systemic change, the program addresses both immediate needs and long-term
goals. It serves as a reminder that disability inclusion is not just a policy issue but a
shared responsibility to ensure that everyone, regardless of ability, can thrive and
contribute to their fullest potential.

II. OBJECTIVES

1. Raise Awareness: Educate the public about the rights, abilities, and challenges of
persons with disabilities to combat stigma and promote the social model of disability.

2. Promote Inclusion: Foster an inclusive environment by encouraging equitable


access to education, employment, healthcare, and community activities for PWDs.

3. Empower Individuals: Provide PWDs with tools, resources, and opportunities, such

4. Celebrate Achievements: Highlight the contributions and successes of persons with


disabilities in various fields to inspire and recognize their impact on society.

III. METHODOLOGY

The methodology for a PWD Week Program emphasizes participatory planning,


accessible implementation, and impactful evaluation to ensure the program meets its
goals of empowering PWDs, raising awareness, and fostering inclusion. By centering
the voices of persons with disabilities and leveraging community collaboration, the
program can create meaningful, lasting change while celebrating the diversity and
contributions of PWDs.

IV. DATE AND VENUE

The activity is scheduled to take place in July 2025 at MSWD office in


Buluan, Old Kaabakan Municipality.

V. BENEFICIARIES

The beneficiaries of the PWD week programs are those who are identified as
deserving PWDs in the community.

VI. BUDGETARY REQUIREMENTS

The sum of ONE HUNDRED FIFTY NINE THOUSAND TWENTY TWO PESOS
(159,022.00) shall be needed to defray the following expenses for the activity, to wit:

VII. SOURCE OF FUND

The cost needed for this activity shall be charged against the available funds of the
MUNICIPALITY OF OLD KAABAKAN PERSONS WITH DISABILITIES FUND subject
to the usual accounting and auditing rules.

VIII. MECHANICS

Activity Expected Outcome

Announcement and Barangay officials, in coordination with MSWDO, will

Verification of Eligibility Applicants must be officially recognized as a PWD by


presenting a valid PWD ID or a medical certificate.

Those without a PWD ID must secure a medical certification


from an accredited health professional.

Verification and
Endorsement Barangay focal persons will conduct an initial review of
submitted documents.

Incomplete applications will not be accepted and must be


returned to the applicant for completion.

Endorsement to the Barangay officials will consolidate the verified applications


Municipal Social Welfare and endorse them to the MSWDO for further processing.

A summary report of applicants per barangay must be
submitted along with the documents.

Validation by MSWDO The MSWDO will review and validate all endorsed
applications.

Additional verification may be conducted for applicants with


missing or unclear details.
