GUIDELINES TO WRITING A FAMILY CASE STUDY

Eligible cases for community case study


 Newly diagnosed chronic conditions 6 months and below
 Cerebral palsy
 Malnutrition
 Growth / Mental retardation
 Disability – Congenital abnormalities
 Substance abuse
 Jiggers

Sources of clients for community / family case study

 Hospital wards /clinics


 Occupational therapy department
 Community
 Churches
 Neighborhood
Once the case has been approved for follow up then
 Attach a duly signed copy of the patients notes if the hospital is your source of referral
 Write an abstract of your 1st encounter with the client
 State specific reasons why you think that client needs follow up
 The following procedure should be followed
1. Comprehensive History and detailed Physical Examination as per the guidelines in the
NCK procedure manual ( We have for pediatrics and adults )
2. They also should include:
a. Vital signs
b. Weight / Height
c. BMI – for adults and interpret the findings
d. MUAC – For children nutritional assessment – interpret findings
e. Investigations done and their findings
f. Current treatment the client is on
3. Literature review on the topic under study. Should include in text citation and
reference (Harvard)- introduction, definition, pathophysiology, aetiology,
risk/predisposing factors, classification, diagnosis , management – medical
/pharmacological, nursing mx using Nursing process, collaborative mx), complications,
patient education and follow up and references
4. Previsit Activities- This should include
a. Previsit objectives
b. Previsit report to include
i. Geographical location of the clients home
ii. Carry out home assessment using the guideline in the students training file
iii. Health services available to the family
iv. Problem list identifiedbased on your findings during History, Physical
examination and Previsit assessment
v. Catageorize the identified problems either as family health threats ,
health deficits or foreseeable crisis
vi. From each category identify the problems that you can assist the family to
solve
vii. Nursing diagnoses for each problem identified in each category
viii. Objectives for follow up based on the nursing diagnoses that have been
selected
ix. Family Nursing care plan to be implemented during follow up
x. A work /Action plan for follow up based on the care plan –should include
resources needed
5. Actual home visits
a. Should be as per the objectives and the work plan
b. Include the activities to be carried out during each visit
c. Include lesson plan for each visit
d. Reasons for termination of the visits
6. Achievements as a result of the home visits
a. To the family
b. To the student
7. Conclusion
8. Recommendations
9. References
