CPT1 CAMPUS

APE PRE-REGISTRATION FORM FOR NON-AGENTS


GENERAL REMINDERS BEFORE APE

1. ALL patients should fast for 8 hours to 10 hours upon arrival at the registration
area to meet the maximum of 10hrs fasting requirement during blood extraction.
Consider the queuing time at the registration area to avoid overfasting;

2. For FEMALE PATIENTS who opt to take Pap Smear exam:


No sexual contact 3 days prior the examination
The last day of menstruation should be at least 10 days prior to the
examination date

3. Patients with ECG in their package:


Refrain from putting lotions and creams

4. Patients who will undergo Chest X-ray:


Remove all jewelries prior to the x-ray
Avoid wearing shirts with buttons
Females are encouraged not to wear undergarments with underwire

5. Claim your stool specimen cup from our site clinic 2-4 days before the
scheduled APE.
The patient should collect a pea-sized stool specimen before proceeding to
the on-site APE and must submit it to the APE staff within 2 hours after
collection.
GENERAL REMINDERS DURING APE

1. Refer to the attached APE flow for the step by step procedure.

2. Please bring two IDs (1) Company ID AND ( 2) Any valid government ID.

3. All registered patients are required to complete the APE on-site.

4. Completion of APE offsite and after the schedule are only allowed for FEMALES
with menstrual period (for urinalysis and pap smear) and or suspected
pregnancy (for chest X-ray). HRS will release the guidelines for those who are
not able to complete the procedure onsite.

5. Exams not completed on site other than conditions in item 4 will be taken at
any Aventus Clinic for Manila employees and Aventus Cebu Cybergate Clinic or
Aventus Cebu IT Park for Cebu based employees and expenses shall be shouldered
by the employee except those who will be able to submit a valid justification.

6. For FEMALE patients:


Physical Exam must be accomplished prior to having the Chest X-ray;
A waiver must be signed prior to the Chest X-ray procedure.;For patients
delayed menstrual cycles or are suspected pregnant, please inform the
mobile team.

7. Submit your stool specimen to the APE staff within 2 hours after collection.

8. Steps in urine collection for urinalysis:


Use an approved empty container for collection.
Wash and dry your hands thoroughly.
Remove the container cap and set it aside. Do not touch the inner surface of
the container to avoid any possible contamination.
Wash your urogenital area (lower parts) with a toilet paper or a mini towel.
Collect the midstream during the urination process.
Place the cap and close properly.
Wash your hands after collecting the specimen.
Bring the specimen to the medical technicians for submission.
CONSEQUENCE FOR NON COMPLIANCE

In compliance with Department of Labor and Employment (DOLE) Occupational


Health Services regulations, all employees are required to undergo the Annual
Physical Examination (APE).

For this year's APE, all eligible employees must adhere to their designated schedule
and site assignment. Off-site APE requests will not be granted (refer to FAQs for
exceptional case). Failure to comply or appear during their designated schedule
without valid justification will result in disciplinary action, in accordance with our
code of discipline.
