DEFINITION
Cleansing patient's external genitalia and surrounding skin using, antiseptic
solution.
PURPOSES
1 To cleanse the perineal skin.
2 to reduce chances of infection of episiotomy wound.
3 To stimulate circulation.
4 To reduce body odour and improve self-image.
5. To promote the feeling of wellbeing,
EQUIPMENT
1. A clean tray containing:
a. Sterile antiseptic lotion-2% Dettol or Savlon.
b. Sterile normal saline in a bottle
c. Cheatle forceps.
d. Antiseptic or antibiotic medication if ordered.
e. Sterile sanitary pad.
f. Kidney tray.
g. Sterile gloves.
h. Mackintosh
2.Sterile pack or tray containing
a. Artery forceps-1.
b. Dissecting forceps-1.
c. Cotton balls.
d. Gauze pieces.
e. Sterile towel to wipe hands after surgical scrub.
3. Additional items
a. Infrared light.
b. Bedpan (if procedure is done at bedside),
c. Jug with warm water.
d Receptacle for used perineal pad,
e. Face mask.
PROCEDURE
NURSING ACTION RATIONALE
1.Identify client's need for the perineal
care

1. sterile perineal care pack
containing-
-Cup with cotton balls
-Artery forceps
-Thumb forceps
2. Other articles-
-Disinfectant solution
-K-Basin
-Extra cotton balls
-Sanitary pads
-Hip tie
-A pair of gloves
4.Prepare environment and provide Provides privacy and reduces

5.Position a mackintosh under the

majora with non-dominant index finger
and thumb
16.Clean with cotton ball and antiseptic Cleaning from more cleaner
solution from clitoris down. Use fresh area to least clean area prevents
cotton ball for each stroke contamination
17.Then clean labia minora and labia
majora using one cotton ball for each
stroke
18.If catheter is present, clean urethra
around catheter using fresh cotton ball.
Clean catheter to a length of 2 inches
away from urethra using fresh cotton
ball
19.Clean perineal suture line if present
20.Finally, clean from forchette to anus
21.Fix pad from front to back
22.Put back the draw sheet or replace if
soiled
23.Position patient comfortably
24.Wash hands
25.Replace the article
26.Record the procedure- date, time,
type of lochia, colour consistency, odour
and condition of the suture line if
present.
27.Report any foul smelling discharge
and gaping of wound to the concerned
doctor
SPECIAL CONSIDERATIONS
1. If a sitz bath is prescribed, give it before perineal care.
2.If patient has urinary catheter, provide catheter care along with perineal
care.
Summary
Today we learned about perineal care, purposes, equipment’s and procedure
of perineal care.
Conclusion
Maternal and health- care related factors were associated to the prevalence
and degree of spontaneous perineal care.it is imperative that women receive
high quality evidenced based care wherever childbirth takes place.
Practice that reduce the adverse effect of perineal trauma and make vaginal
birth more desirable are to be encouraged.
