Write up: Chest Pain
The patient is a male, presented with a chief complaint of chest pain for the
past 5 minutes. He reports it as a 6/10. The patient appeared alert and
oriented, was ambulatory, and a reliable historian regarding their health. The
patient reported the chest pain as moderate, localized to the middle of the
chest, and described it as tight and uncomfortable. Pain is aggravated by
movement. Patient has low stress and a sedentary lifestyle. Denies the
history of anxiety or panic attacks. Heart and lung sounds were clear and
normal upon auscultation. The patient denied smoking, alcohol is consumed
2-3 times per week, and no drug use and reported no significant family
history of gastrointestinal disorders. Reports high blood pressure and takes
Lisinopril – 20mg daily. Takes high cholesterol medication, atorvastatin 20mg
daily. Recently had an EKG and stress test with normal results. Denies
palpitations, swelling, circulation problems, blood clots, or bruising. Has
familial history of heart attack but no history of stroke or pulmonary
embolism.
