PRE-ENROLLMENT & ENROLLMENT
1. DOCUMENTS & REQUIREMENTS
1.1. What paper size should be used for printed documents?
1.1.1. Documents should be printed on LETTER (SHORT BOND) or A4.
1.2. Should I prepare an Original Copy (F138) or CTC for my report cards (G10-
11)?
1.2.1. If you still have your Original Copy (F138), then you may submit that to
PUP.
1.2.2. If you no longer have your Original Copy, you may Request a CTC of that
grade in your respective JHS/SHS.
1.3. Can I submit a CTC for Grade 12?
1.3.1. NO. Only Grades 10 & 11 are allowed to be CTC. For Grade 12, you need
to submit the ORIGINAL COPY.
1.4. Can I request my Grade 10 Report Card from my SHS?
1.4.1. If your SHS will allow document withdrawal, then you may.
1.4.2. If not, then request a CTC from your JHS.
1.5. Do my F138s (Grade 10-12) & GMC require dry seal?
1.5.1. YES.
1.5.2. For GMC
1.5.2.1. GMC is required to have the school dry seal.
1.5.3. For F138 — Grade 12
1.5.3.1. The original Copy G12 Report Card is required to have the school
dry seal.
1.5.4. For F138 — CTC (Grade 10-11)
1.5.4.1. The CTC of G10-11 Report Card is required to have the school
dry seal.
1.5.5. For F138 — Original Copy (Grade 10-11)
1.5.5.1. While it does not indicate that it needs to have the school dry seal,
it is ideal to request the school dry seal.

2. MEDICAL
2.1. Do we fill in the Health Information Form?
2.1.1. YES. You will fill in the information in HIF.
2.1.2. BUT, leave the following BLANK:
2.1.2.1. Student ID

2.2. The clinic requires a referral form for X-ray, where can I get one?
2.2.1. You may print & fill out this form: REQUEST FORM CHEST X-RAY
2.3. I already had a chest X-ray, can I submit that?
2.3.1. DEPENDS. Validity of the Chest X-ray result should not be more than 6
months from the enrollment date. If it exceeds the 6-month validity, then
you are required to take another chest x-ray.
2.4. Do we need to get other medical (CBC, Urinalysis, Stool, Etc.) from other
clinics?
2.4.1. NO. You will only provide the X-ray (Photocopy of Results & Film). The
other medical will be done in PUP.
2.5. I’m PWD, do I need additional requirements/documents for medical?
2.5.1. YES. Prepare the following documents:
2.5.1.1. PWD ID (Photocopy)
2.5.1.2. Medical Certificate from your Physician
2.6. I currently have an existing co-morbidity, do I need additional
requirements/documents for medical?
2.6.1. YES. Prepare the following documents:
2.6.1.1. Medical Certificate from your physician, indicating your co-
morbidity & the student is fit to enroll.
2.7. I currently have an existing pulmonary condition, do I need additional
requirements/documents for medical?
2.7.1. YES. Prepare the following documents:
2.7.1.1. Chest X-ray Film & Results (+1 Photocopy of Results)

4. ID
4.1. When is the picture taking for ID?
4.1.1. Picture-taking will happen on the same date of enrollment. After receiving
your Registration Certificate (Step 4), you’ll have your picture taking for
ID (Step 5). *Refer to Route & Approval Slip
4.2. Do we have a yearly picture taking for ID?
4.2.1. NO. Your ID is your ID for the rest of your stay in PUP.
4.2.2. But there will be a REVALIDATION of ID. The revalidation will only
be a replacement for STICKER.
4.2.3. HOWEVER, you will change your ID once you shift programs.
4.3. What are the guidelines for ID?
4.3.1. Attire
4.3.1.1. Wear a NON-WHITE & COLLARED SHIRT, and/or T-
SHIRT.

4.3.2. Hair, Make-up, & Accessories [FOR ID]
4.3.2.1. There are NO hair restrictions in terms of hair dye, as long as you
look presentable.

5.1. When is the Aptitude Test?
5.1.1. Currently, there are NO ANNOUNCEMENTS YET, kindly wait for the

5.6. Is the use of cell phones allowed during the Aptitude Test?
5.6.1. Phones were NOT ALLOWED during the Aptitude Test of Batch 2023.

UNIVERSITY & COLLEGES
1. SLOTS
1.1. Can I secure a slot in [Degree Program]?
1.1.1. We cannot guarantee that you can secure a slot in your desired program. It
will highly depend on your enrollment date and the maximum slots (slots
depend on the allotted budget of the college and its respective
departments).
1.1.2. We highly recommend listing down at least five (5) Degree Programs
before the enrollment period, so you may have a variety of options if ever
your priority degree program is full.
1.2. How will I know if my target degree program is full?
1.2.1. Most colleges will announce the slots left in a degree program via. their
Student Council FB page.
1.3. What programs are quickly filled?
1.3.1. It will depend on the trends during enrollment. But these are the programs
that often fill up quickly (based on the previous year’s trend):
1.3.1.1. Pre-Med Related Programs
1.3.1.2. Engineering (Especially Civil)
1.3.1.3. CAF Degree Programs (BSA, BSMA, BSBA-FM)
2. SHIFTING
2.1. Can I shift to [Degree Program]?
2.1.1. DEPENDS. Shifting will depend on the college & program. Inquire the
chairperson if shifting is allowed after 1 academic year.
2.2. Can I shift to a Boards Degree Program if I’m from a Non-boards Degree
Program?
2.2.1. NO. Only board degree programs are allowed to shift to a non-board
degree programs.
3. LEARNING ENVIRONMENT
3.1. What is our setup for classes? Do we have F2F? Online Classes?

3.1.1. PUP has a Hybrid Learning Modality (In-Person Classes, Online
Classes, and Asynchronous Classes).

4.2.1. DEPENDS. It will be based on your college/program’s guidelines.
5. NSTP
5.1. I have the wrong NSTP selected during the confirmation period, can I
replace it on enrollment?
5.1.1. During the enrollment process, ASK THE INTERVIEWER. It is also
possible for the NSTP Office to post the guidelines for changes to NSTP.
5.2. How many semesters will I take my NSTP?
5.2.1. You will take your NSTP during your 1st year of residency (2 semesters).

6.1. Is there a student handbook?
6.1.1. YES. PUP STUDENT HANDBOOK
7. SIS PORTAL
7.1. I’m an upcoming freshie, should I open my SIS portal?
7.1.1. NO. It is NOT RECOMMENDED to open your SIS Portal YET, as your
enrollment process for 1st Year-1st Semester will occur face-to-face
(F2F). This is to avoid causing internet traffic for the higher batches.
7.2. When can I open my SIS portal?
7.2.1. Ideally a week before the first day of school.
7.2.2. You may also open your SIS portal:
7.2.2.1. After encoding of grades;
7.2.2.2. If you have tagged professors;
7.2.2.3. Semestral Enrollment Period (After 1st Yr - 1st Sem)
7.3. What is the SIS portal?

8.3. Can I join this organization if it’s under the jurisdiction of a college different
from mine?
8.3.1. If the organization is U-WIDE, then yes. If not, then you may not join the
organization.
8.4. I plan to join the college’s student council, how can I apply?

1. TERMINOLOGIES
1.1. College
1.1.1. Pertains to the group of related degree programs. (Eg. College of
Engineering [CEng], College of Education [CoEd], College of
Accountancy & Finance [CAF])
1.2. Degree Program
1.2.1. Pertains to the academic degree. (Eg. BSIT, BSBA, BSCS, BSPS, Etc.)
1.3. Course
1.3.1. Subjects within the Degree Program (Eg. PathFit, ObliCon, FAR, Etc.)
1.4. Adjustment Period
1.4.1. It is when:
1.4.1.1. Tagging of Professors;
1.4.1.2. Enrollment of Returnees, Transferees, & Shiftees
1.4.1.3. Return of LOAs
1.4.1.4. Orientation Purposes
1.5. Departmental Examinations (Deptals)
1.5.1. Deptals are the equivalent of MAJOR EXAMS. Oftentimes it is the
highest percentage of the subject grade (30-60%).
1.6. Recits
1.6.1. Recits is short for RECITATION.
1.7. Grading System
1.7.1. The grading system will based on the PROFESSOR. They will provide an
outline of the grading system of their subject, and it will always be
different from other professors.
1.8. Dos Policy
1.8.1. Only applicable to COLLEGE OF ACCOUNTANCY & FINANCE
(CAF)
1.8.2. A student in CAF should maintain a grade higher than 2.00 in all major
subjects, to be exempted in Special Qualifying Exam (SQE).
1.8.3. If a student has a grade lower than 2.00, they are required to take the SQE,
which will determine if they may proceed to the next year.
