JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, LORETO J., of legal age, and a Brgy. Captain of Poblacion Norte, Sigma, Capiz
and REX F. , of legal age, and Brgy. Secretary of Poblacion Norte, Sigma, Capiz, Philippines,
after having been duly sworn to in accordance with law, do hereby depose and say, THAT:

1. We personally know JOHNBERT, the son of NORBETO J.. and MA. LEDDA, who is a
resident of Brgy. Poblacion Norte, Sigma, Capiz;

2. We know for a fact that the JOHNBERT was born on 17 March 2XXX in Poblacion
Norte, Sigma, Capiz;

3. It appears in the Certificate of Live Birth of JOHNBERT, the full name of his father is
NORBETO J.. However we know for a fact that his father’s true and correct name is
NORBETO J. JR., as shown by his Voter’s Identification Card and Barangay ID,
attached hereto;

4. We further attest that NORBETO J. and NORBETO J. JR. refer to the same person;

5. We are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in this
matter whatsoever;

6. We are executing this affidavit to attest the truth of the foregoing and for whatever
legal intents or purposes it may serve.
