Questionnaire on Environmental Issues
Purpose: To gather community opinions on solutions for environmental issues. Your
answers will help identify which solutions are most supported by local residents.
Instructions for Completing the Questionnaire
How to Respond: Simply circle or check "Yes" or "No" for each question.
Data Collection: Ask the following questions from one person
(friend,neighbour,family etc)Simply circle the responses.
