1 Purpose
To describe the procedure that ensures the health and safety of all persons who enter or work in
confined spaces.
Known confined spaces are listed in the ‘confined space register’ for each worksite. They also include
any space identified as a confined space through a job hazard analysis (JHA) done for a particular job.

2 Scope
This procedure applies on BNITC work sites and is to be used if confined space entry is required. Hazard
and Risk assessment shall be completed and the risks controlled by use of the permit to work and/or a
confined space permit which is issued to assist in the identification and control of hazards and risks
associated with each confined space.

3 Roles and Responsibilities


3.1 PROJECT MANAGER
 Provide employees with comprehensive and relevant information pertaining to the risks
associated with confined space entry that exist in the workplace and on any control measures
that are in place to reduce those risks
 Ensure that employees’ capabilities have been assessed prior to allocating work in a confined
space.
 Ensure that employees associated with confined space entry operations are briefed by the
confined space entry supervisor of the hazards and risks associated with confined space entry
operation
 Ensure that a Method Statement, Risk Assessment, Job Safety Analysis are in place prior to each
confined space entry activity
 Ensure employees involved with confined space working communicate/understand the same
language
 Implement a Permit to Work system in a Confined Space
3.2 CONSTRUCTION MANAGER
The Construction Manager is accountable to provide the necessary resources to ensure safe systems of
work that ensure this procedure is understood, applied and observed within their area of authority.
3.3 HSE MANAGER
The HSE Manager is responsible for:
 Ensuring that Confined Space training is conducted and of an appropriate quality.
 Ensuring safety systems are kept up to date in relation to this procedure.
 Recording and analyzing incident/investigation data.
 Ensuring that this procedure is reviewed as stipulated.
3.4 SITE ENGINEER
The Site Engineer shall ensure this procedure is duly followed by personnel working under their control.
He shall be responsible for the safety of personnel by following this procedure and for maintaining
appropriate records relating to their activities.
3.5 EMPLOYEE
The Employee is required to act in a manner that does not adversely affect their own health and safety,
or that of others. They may be instructed persons and shall comply with the terms of this procedure

and must immediately report to the accountable person any matter that may affect their own or others'
health and safety.
3.6 CONFINED SPACE ENTRY AUTHORISED PERMIT ISSUER
Confined Space Entry Authorised permit issuer shall:
 Prepare and issue Confined Space Entry Permits and shall accept the responsibilities associated
with their authorisation.
 Have the authority to issue and withdraw any permit at any time when a permit condition is
breached. It is mandatory that the Permit to Work Issuer inspect the defined confined space
area at site before issuing a confined space entry permit to ensure that all safety measures have
been taken by the Confined Space Entry Supervisor.
 Inspect the defined confined space area at site before issuing a confined space entry permit to
ensure that all safety measures have been taken by the Confined Space Entry Supervisor.
3.7 AUTHORISED GAS TESTER (AGT)
 Authorised Gas Tester shall ensure the use and interpretation of each instrument used for gas
testing and appointed by the Construction Project Manager to perform confined space and hot
work atmosphere testing
 Update his knowledge on the hazards that may be faced during entry, including informationon
the mode, signs and symptoms, and consequences of the exposure;
 identify all chemical hazards and recommends control measures to ensure safe entry;
 be familiar with all conned space entry equipment used, for example emergency breathing
apparatus, man riding winch, tripod, etc.;
 Test and interpret the reading level of gaseous in the confined space.
3.8 CONFINED SPACE ENTRY SUPERVISOR
Confined Space Entry Supervisor shall ensure the following prior to entry into a confined space.
 There is no presence of toxic gas inside the confined space
 Artificial ventilation and lighting are in place
 Workers entering into confined space are experienced, trained and certified
 Confined Space Entry Attendant is present
 Confined Space Entry Permit is issued
 Permit to Work is issued
 Suitable/Sufficient access and egress
 Adequate and intrinsically safe lighting
 All permits and the names of personnel who have entered the confined space are displayed
outside the confined space in a conspicuous location
 Fire extinguisher is present (if required in a confined space)
 A first aid kit is present
 Communication facility is active
 Suitable and sufficient PPE is used
 The atmosphere is not explosive, with flammable levels less than 10 percent of the Low
Explosive Limit (LEL)
 The concentration of toxic materials is less than the occupational exposure standards specified
 The external atmosphere is safe from toxic materials
 There is compliance with hygiene standards imposed by codes of practice on specific materials,
where applicable
 The oxygen concentration is in the range 19.5 percent to 23.5 percent per volume

 The oxygen content of the air both inside and outside the space is measured
 The temperature in the confined space is not or will not become hazardous
 Emergency and rescue team is available and properly trained
3.9 CONFINED SPACE ENTRY ATTENDANT
The roles and responsibilities of confined space entry attendant are as follows:
 shall remain at the entrance of the confined space throughout the entire period of the
confined space work (a confined space entry attendant shall not leave his station for
whatever reasons unless another confined space entry attendant is on duty
 To keep a close watch on the atmospheric conditions inside the confined space and
communicate any changes observed.
 Know the behavioral effects of the hazards.
 Be able to identify the authorized entrants.
 Communicate with entrants throughout the work period.
 Monitor and evacuate entrants if necessary.
 Summon rescue, if needed.
 Warn away unauthorized person

5. Definitions and Abbreviations


The definitions of the terms and acronyms used in this Specification are given below. Some of the terms
and definitions are not universal and are exclusively defined for the CITY UNIVERSITY COLLEGEL.
 AUTHORISED GAS TESTER (AGT): a person trained in the use and interpretation of each
instrument used for gas testing and appointed by the Construction Project Manager to perform
confined space and hot work atmosphere testing
 COMPETENT PERSON: a competent person is someone who, through training and experience, is
knowledgeable of the various Qatari Laws and HSE standards that apply to their workplace, is
capable of identifying workplace hazards associated with various construction activities and has
the authority vested in him to execute the required corrective actions
 CONFINED SPACE ENTRY ATTENDANT or HOLEWATCH: A person trained and tested to a
practical knowledge level of Confined Space Entry and work procedures, who shall remain at the
entrance of the confined space throughout the entire period of the confined space work (a
confined space entry attendant shall not leave his station for whatever reasons unless another
confined space entry attendant is on duty)
 CONFINED SPACE ENTRY AUTHORISED PERMIT ISSUER: a person trained and tested to a
practical knowledge level of confined space entry permit procedures (the Permit Issuer shall be
authorised in writing by the Construction Contractor's Project Manager to prepare and issue
Confined Space Entry Permits and shall accept the responsibilities associated with their
authorisation)
 CONFINED SPACE ENTRY SUPERVISOR (Entry Permit Recipient or Acceptor): a person, who
determines that the acceptable entry conditions are present at a permit space where entry is
planned, authorises entry, oversees and manages entry operations (the Confined Space Entry
Authorised Permit Issuer and Confined Space Entry Supervisor shall not be the same person.
However, a Confined Space Entry Supervisor may serve as an attendant or an authorised
entrant, as long as he is trained and equipped as required for each role he fulfils. Also, the
duties of entry supervisor may be passed from one individual to another during the course of
an operation in a confined space
 CONFINED SPACE WORKER: an employee trained and certified by a third party for working in a
confined space

 CONFINED SPACE: a space with limited means of access or egress which is not intended for
continuous human occupancy and contains or has the potential to contain a physical or
atmospheric hazard that may lead to human life at risk of death, incapacitation and impairment
of ability to self-rescue, injury or acute illness
 CONTRACTOR: A Construction Contractor appointed by DOHA BRITISH SCHOOL for execution of
the project.
 CSEP: Confined Space Entry Permit
 HSES: Health, Safety, Environment and Security
 HSESMS: Health, Safety, Environmental and Security Management System: OHSAS: Occupational
Health and Safety Assessment Series
 INCIDENT: Incident is an undesired and an unplanned event that results in or could have
resulted in fatality, personal injury, property damage, environmental harm or a combination of
all the above stated events
 PERMIT TO WORK ISSUANCE DEPARTMENT: The Construction Contractor's Department within
the Project's organisational structure, staffed with experienced and competent personnel,
assigned and appointed by the Construction Contractor's Project Director to administer and
issue all related permits
 PPE: Personal Protective Equipment
 PROJECT: Project is the total process of design, documentation, approvals and execution. It
involves construction of roads, bridges and other associated works
 QCS: Qatar Construction Specification
 SITE OFFICE: an administrative office established within or outside the project.
 STSI: Specific Task Safety Instructions
 TBT: Toolbox talk
 THIRD PARTY: any establishment other than the manufacturer and the owner of equipment or
plant recognised by the Qatar government agency to certify plant/equipment or any
establishment other than the Contractor and Sub Contractor recognised by the Qatar
government agency to provide training to the workforce.
 WORK ENVIRONMENT: the locations within or outside the ROW of a project, where an
employee is present for the execution of his specified duties as directed by a Construction
Contractor or Subcontractor
 WORK SITE: the work environment within and outside of a project, where an employee is
present for the execution of his specified duties as directed by a Construction ,Contractor or
Subcontractor; or the land and other places under in or through which the works are to be
executed or carried out and any other land or places provided by the DBS or Qatar government
for the purpose of the project together with such other places as may be specifically designated
in the Contract as forming part of the site.

6. Procedure
A. DOCUMENTATION
The required documents associated with confined space are shown below.
The Confined Space Entry Supervisor shall keep a copy of all these documents:
 A Method Statement
 Risk Assessment
 Job Safety Analysis
 Permit to Work
 Confined Space Entry permit
 Entry Register
 Any other permits / Isolations from the local administration or the utility service provider (if
required)
B. PROCEDURE FOR ENTRY INTO CONFINED SPACE
1. If the Entry Supervisor intends to have any work activity carried out in a confined space, he shall
consult with the Confined Space Entry Authorised Permit Issuer.
2. The Entry Supervisor shall present a formal Confined Space Entry Method Statement to the
Permit to Work Issuer in writing confirming the date, time, types of activities, number of
personnel working or associated with the activity.
3. The Confined Space Entry Authorised Permit Issuer and the HSE Manager or his nominee shall
assess all of the intended activities to identify activity specific risks.
4. Confined Space Entry Authorised Permit Issuer shall have the authority to issue and withdraw
any permit at any time when a permit condition is breached. It is mandatory that the Permit to
Work Issuer inspect the defined confined space area at site before issuing a confined space entry
permit to ensure that all safety measures have been taken by the Confined Space Entry
Supervisor.
5. It is mandatory that the Permit to Work Issuer inspect the defined confined space area at site
before issuing a confined space entry permit to ensure that all safety measures have been taken
by the Confined Space Entry Supervisor.
6. Permit to Work Issuer shall inspect the work area every 2 (two) hours and ensure that all the
required safety measures are in place.
7. Before workers enter a confined space, the Confined Space Entry Supervisor shall conduct, in the
presence of the Permit Issuer, a STSI meeting with all concerned to communicate and establish
effective lines of authority and communication in order to minimise any risk of subsequent
misunderstanding.
8. The Confined Space Entry Supervisor shall ensure that all concerned have been provided with
the necessary PPE including respiratory protective equipment if required (all respiratory
protective equipment must be inspected by a competent person).
9. The Confined Space Entry Supervisor shall ensure that continual forced ventilation is operational
at least 30 minutes before any person enters the confined space and the forced ventilation shall
be operational throughout the work activities.
10. The copy of Permit to Work, permit to enter and names of personnel who have entered into the
confined space shall be displayed outside the confined space in good visibility throughout the
duration of the confined space operation.
11. The Permit to Work shall show the number of workers working inside a confined space.

12. The assigned Entry Attendant(s) shall be briefed for the specific confined space entry and shall,
for the entire duration of the job, be in attendance outside at the entrance of the confined
space.
13. The Confined Space Entry Supervisor shall ensure that the power supply into the confined space
shall not be interrupted.
14. Testing of the internal atmosphere of a confined space shall be carried out as near as practicable
to the time of entering into a confined space. Ref - J &P ORBPO23-F 45
15. Testing shall be repeated as circumstances dictate throughout the duration of the confined
space operation.
16. The AGT shall confirm that the sampling or testing is carried out in a safe manner as specified on
the Confined Space Entry Permit and the reading shall be logged in the Gas Testing Register.
17. The AGT shall ensure that the sampling or testing equipment used in confined spaces is
maintained in good working condition and is calibrated in accordance with the manufacturers'
Specifications and requirements.
18. Maintenance, servicing and calibration records shall be kept in the Permit to Work issuance
department office and shall be made available to the Engineer or Engineer's Representative
upon request.
19. Continuous monitoring shall be achieved by the wearing of a personal monitor by persons
entering the confined space or the provision of a fixed monitoring device.
20. In the event of significant interruption to the work under any circumstances, the atmosphere
shall be retested before re-entry.
21. In the event that a monitor alarm is activated or other emergency stops the work being carried
out, the workers shall discuss the conditions with the Authorised Confined Space Entry
Supervisor before recommencing work.
22. Ensure communication is via one agreed language which is understood by all personnel involved
in confined space work activities. Communication; While the work is in progress the erection
supervisor shall maintains frequent communication with the personnel inside through visual
means, if possible, or by other means such as two-way radios or hand signals. After the
completion or suspension of work inside the space, all personnel must sign off the confined
space permit as soon as practicable. The Site Engineer shall ensure that all personnel have
vacated the space and any tools or materials taken in are removed from the space before the
cancellation of the permit.
1. Rescue personnel and equipment such as a Compressed Air Breathing Apparatus (CABA), Lifting
equipment, and safety harness / life lines as a minimum shall be in place at the point of entry
before any person is allowed to enter the confined space.
2. The temperature in the confined space shall be continually monitored as, during hot weather, in
a confined space temperature may rise and can result in high potential risk to personnel.
3. A first Aid kit and suitable fire equipment shall be in place
4. Necessary number of blowers 1
5. Generator or a source of uninterrupted power supply
6. Various types of the required PPE

PERSONNEL
1. The Project Manager shall appoint all personnel except the confined space workers in writing.
2. Personnel entering into a confined space shall be free from any hearing or visual impairment.

3. Personnel entering into a confined space shall be of good health / physically fit and be able to
carry another person in case of emergency.
4. The Project Manager shall ensure that personnel selected for confined space activities are
trained, competent and experienced.
5. Workers who suffer from asthma, bronchitis, other respiratory conditions, or whose immune
system has been negatively affected shall be assessed by a medical practitioner as to their
suitability to work in confined spaces.
6. A first Aider shall be available onsite at all times during confined space activity.

7. Other health conditions which might indicate that a person is not suitable for working in a
confined space, or that further checks need to be made prior to authorising the person to
participate in these activities are:

 partial or complete deafness


 high blood pressure
 lack of mobility through joint problems
 diabetes
 depression or other mental illness
 defective eyesight
 sensitivity of the skin to some substances
 taking some types of medication
When respiratory protection is to be used, it shall be noted that facial and long hair often prevent a
respirator from fitting properly, therefore not achieving an adequate seal or offering the maximum
degree of protection. All personnel working inside a confined space shall not have any type of facial
hair.

7. TRAINING AND AWARENESS


RdE shall give information, instruction and training to employees that enable them to carry out work
safely and without risk to their health.
1. HSE department shall give STSI to all employees associated with confined space operation
2. An entry into a confined space which necessitates the use of breathing apparatus shall require a
full breathing apparatus and rescue training course to ensure competence.
3. It is important that trainees shall be familiar with both equipment and procedures before
initially working in confined spaces.
4. No person shall enter a confined space unless the person is trained and competent.
5. The training needs for each of the following categories of employees to enter confined spaces
shall be identified:
 Confined Space Entry Authorised Permit Issuer
 Authorised Gas Tester (AGT)
 Confined Space Entry Supervisor
 Confined Space Entry Attendant
 Confined Space Entry Worker
The training program shall include as a minimum, the following:
 The hazards of confined spaces
 Assessment procedures
 Control measures

 Emergency procedures
 Selection, use, fit and maintenance of safety equipment & PPE
 Isolation of energy sources
 Confined space entry permit requirements
 Airline Respirator and SCBA training
 Proper mechanical ventilation techniques and atmospheric gas testing

8. ISOLATION
1. The confined space shall be isolated from all possible external sources of danger to personnel
entering into a confined space.
2. There shall be warnings and informative sign boards placed near a confined space.
3. After operations in a confined space, the entry into a confined space shall be closed to prevent any
unauthorised personnel entering the area.

9. CONTROL OF HAZARDOUS ENERGY (LOCKOUT/TAG OUT)


Energy sources including electrical, mechanical, hydraulic, pneumatic, thermal, chemical or other
sources can be hazardous to workers associated with confined space operations. Proper lockout /tag
out (LOTO) practices safeguards workers from hazardous energy release. All employees who work in an
area where energy control procedures are utilised need to be instructed in the purpose and use of the
energy control procedure(s), especially prohibition against attempting to restart or reenergize machines
or equipment that are locked and tagged out. All employees who are involved in confined space
operations and are authorised to lockout machines or equipment need to be trained in recognition of
applicable hazardous energy sources, the type and magnitude of the energy and the means and
methods of isolating and controlling the energy.

10. PURGING AND VENTILATION


Air purging and ventilation shall only be carried out by removing covers, doors etc. and allowing natural
air circulation or forced air circulation to and from the confined space.
1. When an inert gas (such as nitrogen) has been used to purge or render a flammable atmosphere
safe, the inert gas itself shall be properly purged with air and the AGT shall perform the required
tests to ensure the atmosphere in the confined space is safe to enter.
2. When air purging is taking place, the flow of air shall be of a sufficient volume and velocity to
ensure that no pockets or layers of gas remain undisturbed.

11. ATMOSPHERIC MONITORING


1. When the result from a Risk Assessment (RA) indicates the presence of any substance hazardous to
the health of personnel inside the confined space, continuous atmospheric monitoring shall be
carried out
2. Before an entry is made into a confined space, it is mandatory that all tests shall be carried out to
establish the levels of oxygen, toxic or flammable gas in the atmosphere.
3. Maximum level of flammable gas present in any work atmosphere is 10% of the Lower Explosive
Limit (LEL)
4. The minimum allowable oxygen level for safe entry is 19.5 % by volume

5. Maximum allowable level of toxic gases is shown below:


 Ammonia 25 ppm
 Carbon monoxide 25 ppm
 Chlorine 0.5 ppm
 Hydrogen sulphide 10 ppm
 Sulphur dioxide 2 ppm
6. The external atmosphere around the opening of a confined space shall be monitored first by the
AGT and if the results are satisfactory, internal monitoring shall be carried out by lowering a gas
monitor into the confined space before it is occupied.

7. Properly trained and competent personnel shall use simple, reliable instruments to measure
oxygen, flammable and toxic gas levels.

8. The accuracy of the instruments shall be assured by correct calibration and maintenance in
accordance with manufacturers requirements.

9. The human sense of smell shall never be relied upon to detect flammable and toxic gases as some
gases are odourless, i.e. hydrogen sulphide in particular, can paralyze the sense of smell to such an
extent that even fatally high concentrations of the gas cannot be detected.

10. Providing that the specific contaminant is known, tests shall be carried out by the competent
persons using the individual detector tubes available for the detection of specific toxic, asphyxiant
and explosive gases.

12. COMMUNICATION
1. The Confined Space Entry Supervisor shall ensure that continual and effective communications
exist between those inside and those outside the confined space.
2. In the event of an incident, a warning signal can be given and all personnel shall immediately be
evacuated from inside the confined space.
3. The communication system needs to be 'fail safe', ensuring that if a reply is not received or a
scheduled call not made, the procedure for rescue starts immediately.
4. When a confined space is such that the person entering it cannot move far from the entry point
and there are no other factors that could hinder effective communication, the method of
communication shall be pre-arranged to a system of tugs on the safety rope.
5. However, if the nature of the confined space requires the 'entry person' to travel some distance
from the entry point, a more elaborate and effective communication system shall be provided

13. FIRE SAFETY


1. Confined Space Entry Supervisor shall ensure hot work is not carried out in a confined space unless
a valid Hot Work Permit is issued by the permit to work authorised permit issuer, atmospheric
testing has confirmed that flammable or explosive gases are not present and the findings of a risk
assessment show that it is safe to do so.
2. If hot work is in progress in a confined space, the Confined Space Entry Supervisor shall ensure that
there is no residual risk of fire and in such cases a suitable fire extinguisher shall be kept at the
confined space entry point and inside the confined space as well.

3. If CO2 fire extinguisher is activated in a confined space to extinguish fire, then no work shall be
carried out until re-tests are carried out to ensure a safe work environment exists inside the
confined space.
4. The local fire service (Qatar Civil Defence) shall be called in the event the fire cannot be
extinguished in the confined space.

14. EMERGENCY RESCUE PROCEDURES FOR CONFINED SPACE


All workers shall be trained on rescue plans before entry or work in a confined space.
The rescue procedure shall include: Names of the designated rescue personnel clearly displayed and
communicated to all workers on site;
The methods of rescue to retrieve persons inside a Segment /confined space; Hand-cranked mechanical
winch and tripod shall be used to retrieve personnel from inside segment to the top of deck. Injured/
unconscious worker shall be rescued from top of the deck using Man basket/mobile crane or
Mechanical Elevating Work Platform. (MEWP)
This method shall prescribe the types and availability of equipment necessary for rescue; and provide
an effective recall means to summon the designated rescue personnel in a timely manner.
Rescues shall be well planned and specific for each type of confined space. A rescue drill in a confined
space shall be held at least once in every 6 months to familiarize all workers with the use of rescue
methods and equipment. Rescue and emergency equipment shall be suitable and allow emergency
arrangements to be carried out in a timely and safe manner.
Rescue equipment shall include:
 Full body harness with retrieval line attached;
 Hand-cranked mechanical winch and tripod (required when entrant is five feet or more below
the entrance);
 Ladder;
 Stretcher;
 Approved head protection;
 First aid equipment.
 Ropes, harnesses, fall arrest gear, lifelines, lifting equipment,
 Protective clothing and other special equipment shall be provided for use in case of an
emergency. The equipment shall only be used for the purposes for which they are intended, and
appropriate.
 Note: Unplanned rescue, such as: when someone instinctively rushes to help a downed Co-
worker, can easily result in multiple fatalities if there is more than one potential rescuer

15. PERMIT TO WORK


1. Every entry into a confined space shall be made under a valid confined space entry Permit to Work
(PTW), issued by an authorised permit issuer at the permit to work issuance - HSE department.
2. The confined space entry supervisor shall ensure that all required safety precautions have been
taken and provisions made to secure the safety of those entering and working the confined space,
before accepting and signing the Permit to Work.
3. A Permit to Work shall only be issued by a permit to work authorised permit issuer or confined
space entry authorised permit issuer assigned and authorised by Project Manager.

4. It is the responsibility of the Project Manager to ensure those applicable different trades,
(mechanical, electrical, instrumentation, atmospheric testing, etc.) competent persons are assigned
and authorised in writing as authorised permit supervisor (Permit acceptors) to sign a Permit to
Work.
A Permit to Work shall include the following:
 Purpose of entry and identified hazards in the confined space
 Name and signature of the permit issuer
 Name of permit space to be entered
 Names of the authorised entrants
 Name of the confined space entry supervisor and attendant
 Copies of test results with AGT's signature
 Name and signature of entry supervisor who accepted the permit and authorises entry
 Measures taken to manage the identified hazards
 Name and telephone numbers of rescue / emergency services and means to contact them
 Date and authorised duration of work
 Details of additional permits, such as; hot work, that have been issued
 Authorising work in the permit area
 Details of required PPE

16. HAZARD AND RISK ASSESSMENT IDENTIFICATION


The hazards associated with confined space in inspecting, testing, cleaning, repairing and entering
confined spaces include but are not limited to the following;
 Asphyxiation /suffocation or unsafe oxygen level
 Burning or scalding
 Electric shock
 Fire
 Moving machinery
 Noise
 Engulfment
 Explosion
 Poisoning
 Heat Stress
 Falls
 Dust
 Smoke
 Biological Hazards - Contact with Micro-organisms

CONFINED SPACE RESCUE PLAN

The attached On-Site Rescue Plan and these Procedures are part of the written plan for the confined
space and are based on the assessment of hazards in this space.

Prior to entry and/or work in the confined space:

1. The entry supervisor will ensure that the attached “on-site rescue plan” for the confined space
has been completed and that all the rescue equipment identified in the plan is available to effect
a rescue in the confined space.
2. The entry supervisor will ensure that an adequate number of appropriately trained persons (as
documented in the attached “on-site rescue plan”) are available for immediate implementation
of these on-site rescue procedures that apply to the confined space.
3. The entry supervisor will review all emergency procedures, including procedures relating to
emergencies outside the confined space with all entrants and other related personnel.
4. The attendant establishes communication with all workers, using the means described in the
attached “on-site rescue plan”.

On entry and while working in the confined space:

1. The attendant who is stationed outside and near the entrance to the confined space as
described in the attached “on-site rescue plan” remains in constant communication with all
workers inside the confined space.

2. The attendant must be notified immediately if an entrant recognizes:

 unusual action/ behaviours


 an unexpected hazard

 an unsafe act or
 detects a condition prohibited by the permit

3. Entrants must exit the confined space as quickly as possible, when:


 an order to evacuate is given by the attendant or entry supervisor
 an entrant recognizes a sign or symptom of over-exposure –
 an unacceptable condition arises or an evacuation alarm is activated

In the event of a confined space rescue:

1. The attendant does not enter the confined space but immediately summons a rescue response from
the on-site rescue team, using the means of communication described in the attached “on-site rescue
plan”.
