AFFIDAVIT
(RE: Change Body Design of a Motor Vehicle)

I, Padrion, of legal age, single, Filipino Citizen, A resident at Philippines, under


oath declare and state that:

1. I am the owner of the motor vehicle as evidenced by the Deed of Sale a copy of
which is attached, particularly described as follows:

CR No. : 00 MV FILE No.: 138021

2. I modified the old body design Motorcycle with sidecar of the afore described
motor vehicle to MOTORCYCLE WITHOUT SIDE CAR. Thus, the body type of
the above-described motor vehicle is now a “MOTORCYLCE”;

3. I am executing this affidavit to support my request for the issuance of a new


Certificate of Registration with the correction of the body type of the said motor
vehicle from MOTORCYCLE WITH SIDE CAR to MOTORCYCLE with the Land
Transportation Office;

4. Hence, I am executing this affidavit to attest to the veracity of all the foregoing
statements and for all legal intents and purposes it may serve.
