Authorization to Obtain PSA Certificate for my child

To Whom It May Concern,


I, Cassandra Mae S. Gudio, of legal age, residing at 1222 A-Tambunting St. Sta.Cruz, Manila,
hereby authorize my mother Romelita S. Gudio, to process and obtain my PSA Certificate.
Unfortunately, I am unable to personally obtain my PSA Certificate as I have ongoing classes that
I cannot miss. Therefore, I am delegating this responsibility to my authorized representative.
The purpose of obtaining this document is for school requirement.

For identification purposes, I am providing copies of the following:


1. My school ID
2. A xerox copy of my national ID

I confirm that this authorization letter is given voluntarily. Thank you for your assistance.
