NOTICE OF INTERVIEW

The USCIS Quito Field Office is in receipt of the Form I-730, Refugee/Asylee Relative Petition,
noted above. As a next step, a U.S. Citizenship and Immigration Services (USCIS) Officer must
interview you in person to determine your eligibility to immigrate based on your relationship with
your relative in the United States. During the interview, you will be asked to provide evidence of
your identity and your relationship to your petitioning relative. The interview will also serve as an
opportunity for this office to assemble the documentation needed (e.g. passports, photos, required
forms, medical reports, etc.) to continue processing your case.
We have scheduled you for an interview with this USCIS office at the U.S. Embassy/Consulate
located at the street address listed in the upper left corner of this notice.
Your interview is scheduled for April 30, 2025 at 2:00 PM.
Please bring this letter with you on the day of your interview as it will allow you to enter the
USCIS office.
If you are unable to attend this interview, please contact this office as soon as possible to re-
schedule your appointment.
DOCUMENTS YOU MUST BRING TO THE INTERVIEW:
Your case cannot be processed to completion without proper documentation. Please bring to the
interview all the items listed below. For all documents written in a language other than English,
you must also provide a certified English translation. If you are bringing original documents,
please provide a copy that we may keep.
• Medical Examination: The beneficiary must complete a medical examination (which takes 3-4
days to complete) prior to USCIS interview. The medical examination must be performed by an
authorized panel physician. The medical forms will be sent directly to this USCIS office. You
may review a list of authorized panel physicians in Ecuador at

where you will also find information needed to schedule the medical examination appointment.
Note: Once the appointment is completed, the panel physician will transmit the results directly and
securely to the USCIS Quito Field Office. The beneficiary will neither receive a sealed envelope
nor carry any medical forms to the USCIS interview. Medical examinations are generally valid for
six months depending on circumstances specific to the beneficiary. A travel document issued by
USCIS after interview can only be valid until the end of the medical examination's validity period.
Authorized panel physicians can provide anticipated timelines for production and conveyance of
medical reports to USCIS. You may wish to contact the panel physicians directly to learn more.
• USCIS Biometrics Services: Please email [email protected] to schedule an
appointment to come to the U.S. Embassy in Quito for biometrics collection prior to interview.
Failure to schedule a biometrics services appointment may result in additional processing time.
• Inscripción de Nacimiento issued by the Ecuadorian Civil Registry. This birth registration
document must be legalized and stamped by the Civil Registry on the reverse side. If the
beneficiary wishes to retain this document, a photocopy must be provided in addition to the
certified version.
• The beneficiary should bring any other evidence that helps establish that the relationship
between the beneficiary and the petitioner is valid. Examples of the evidence include, but are not
limited to: baptism certificates, family photos, wedding photographs, school records, vaccination
records, correspondence, remittance receipts, or any other personal or official documentation that
establishes the beneficiary's relationship with the petitioner. If the beneficiary wishes to retain
originals, photocopies must be provided as well.
• A legible photocopy of the beneficiary's passport's biographic data page, and copies of all
passport pages with entry/exit stamps, including from prior passports.
• A photocopy of the beneficiary’s national ID (cédula).
• The beneficiary's current residential address, phone number, and e-mail address.
• The petitioner’s current address, phone number, and e-mail address.
• The current address, phone number, and e-mail address of the petitioner’s legal representative (if
any).
• Marriage certificate issued by the Civil Registry, if applicable. If the beneficiary wishes to retain
this document, a photocopy must be provided in addition to the certified version.
• Divorce or Death Certificate, if applicable. If the beneficiary wishes to retain this document, a
photocopy must be provided in addition to the certified version.
Note: Submission of incorrect birth or marriage documentation for the beneficiary may result in a
Service Motion to Reopen this petition and issuance of a Notice of Intent to Deny.
Documents in any language other than English must be accompanied by a certified English
translation.
• Completed Form I-765, Application for Employment Authorization, in the name of the
beneficiary. Complete all items on form, and include all signatures required (including signatures
for interpreter and preparer). The petitioner may sign this form on the beneficiary’s behalf if the
beneficiary is under 14 years of age. Otherwise, do not sign this form on the beneficiary’s behalf.
• Completed Form G-325C, Biographic Information (required for all beneficiaries 14 years of age
and older), to support the Form I-730 before our office. Do not sign this form on the
beneficiary’s behalf. A USCIS Officer will direct the beneficiary to sign this form at interview.
• Copies of any beneficiary criminal records, including (but not limited to) arrest records,
disposition of crimes, court records, records of time served in jail or prison, etc.
Review all requirements and guidance to ensure compliance when submitting the
documentation.
At interview, in addition to the above, the beneficiary must provide the following:
• The beneficiary's physical passport.
• All prior passports issued to the beneficiary.
• The beneficiary’s national ID (cédula).
• Six (6) front-view photographs (51 x 51 mm) of the beneficiary, taken within the past 6 months.
The photographs must be in color with a white background and should be cut to size. Follow

Failure to provide all items above at interview may result in the issuance of a Service Motion to
Reopen your petition and a subsequent Notice of Intent to Deny and/or administrative closure at
the discretion of the local USCIS Field Office Director. If the beneficiary is approved to travel to
the United States, they must be prepared to do so within 50 days of the scheduled interview or
before the expiration of the medical examination, whichever is sooner.
