Return Demonstration Script: Balanced Skeletal Traction (BST)

I. Introduction
Good day, I am [Your Name], a nursing student. Today, I will be demonstrating how to care
for a patient in Balanced Skeletal Traction. This procedure is done to maintain proper bone
alignment, reduce pain and muscle spasms, and promote healing after a fracture. I will
ensure the patient’s comfort and safety while performing this procedure.

II. Preparation
1. Verify doctor’s order for balanced skeletal traction.
✅ Rationale: Ensures correct treatment is being administered.
2. Gather supplies (gloves, traction checklist, documentation tools).
3. Perform hand hygiene and wear gloves.
✅ Rationale: Prevents infection and maintains asepsis.
4. Identify the patient using 2 identifiers (name and birthday).
✅ Rationale: Ensures safety and prevents errors.
5. Explain the procedure to the patient.
“I will check your traction equipment and pin sites to ensure everything is working
properly and you’re comfortable.”

III. Assessment and Care

1. Check the alignment of the body and limb.
✅ Rationale: Misalignment can affect bone healing and increase pain.
2. Inspect traction setup:
- Weights hang freely (not touching the floor)
- Ropes run smoothly through pulleys
- Equipment is not tangled or obstructed
✅ Rationale: Ensures effective and continuous traction force.
3. Assess pin sites for signs of infection:
- Look for redness, swelling, warmth, or discharge
- Clean with sterile saline and gauze if needed
✅ Rationale: Pin site care prevents osteomyelitis and other infections.
4. Perform neurovascular checks (CMS):
- Circulation – Check capillary refill, color, temperature, pulses
- Motion – Ask the patient to wiggle fingers/toes if possible
- Sensation – Ask if there is any numbness or tingling
✅ Rationale: Early detection of complications like compartment syndrome or nerve
damage.
5. Reposition patient as needed without disrupting traction:
- Use trapeze bar if patient can assist
- Support affected limb and maintain line of pull
✅ Rationale: Prevents misalignment and maintains patient comfort.
6. Encourage deep breathing exercises and passive ROM for unaffected limbs.
✅ Rationale: Prevents complications of immobility like pneumonia or contractures.

IV. Documentation
“Document findings such as: pin site condition, neurovascular status, traction equipment
check, and the patient’s pain level and comfort.”

V. Closing
1. Remove gloves and perform hand hygiene.
2. Ensure patient safety:
- Call bell within reach
- Side rails up
- Bed in lowest position
3. Thank the patient and close the procedure:
“The procedure is now done. Please let me know if you feel any discomfort or need
assistance.”
4. Face your evaluator and say:
“That concludes my demonstration of Balanced Skeletal Traction care. Thank you.”
