CHOICE OF CHOOSING A FAMILY PLANNING METHOD.
Scene: Mrs. G is a 39 years old woman, and a housewife. She is G3 P3+0 came to
the clinic with her husband to receive a family planning service. She has not
received any family planning method before and has no knowledge of any family
planning methods. She was referred to Nurse C who is the Head Nurse in charge of
the family planning unit for counselling.
Mrs. G and her Husband entered into the room to see Nurse C
Nurse C: Good morning Sir and ma
Mrs. G: Good morning Nurse
Nurse C: Please you may sit down.
Mrs. G & Husband: Thank you
Nurse C: I am nurse C, how may I help you.
Mrs. G: I am here to receive family planning services
Nurse C: You are welcome, and you are in the right place. Do you have any
particular method in mind?
Mrs. G: No ma that is why I was referred here so that you can educate me.
Nurse C: Feel free to ask any questions, for what so ever we are going to discuss
remains private here.
Mrs. G: Ok ma. (Feeling relaxed)
Nurse C: Do you still want to have more children?
Mrs. G: Yes, but not now.
Nurse C: Here are the various family planning method such as vasectomy and
tubal ligation that suit people who do not want any more children, because it is an
irreversible method, that is, it is permanent.
Mrs. G: Ok ma.
Nurse C: While IUD and Implants suit people who wants more children in the
future but not in the next few years. They are long acting reversible methods,
fertility returns immediately after they are removed, IUDs can last for 5-10 years
while Implants can last for 3-5 years that is Implanon and Levoplant can last for 3
years while Jadelle lasts for 5 years. It requires a simple procedure performed by a
trained provider, it is easily removed and fertility returns immediately but can cause
changes in monthly bleeding. (Pauses)
Mrs. G: Ok
Nurse C: There are also short term reversible methods such as Injectables, pills and
condoms which suit people who do not know how long they want to use
contraceptives. Injectables require taking injections every 8 or 13 weeks, it can
cause changes in monthly bleeding, there can also be a delay in fertility return when
stopped. Pills are only effective if taken with the frequency as instructed while
condoms are effective if used correctly and it is the only method that protects
against STIs and HIV. So, in all these methods that I have explained to you which
one do you choose?
Mrs. G: I choose the 5 years Implant.
Nurse C: Ok, it is fine. Remember that Jadelle Implant has two small rods that are
put directly under the skin of the inner upper arm and it last for 5 years and fertility
returns immediately when removed, it does not prevent Sexually Transmitted
Diseases, it changes monthly bleeding which may be either irregular bleeding
spotting, heavy or no monthly bleeding it can cause headache, acne, breast
tenderness, mood changes, nausea. Most of these side effects will resolve in a few
months but try and come back to clinic if you notice anything unusual like infection
at the site of insertion, expulsion of the rods, failure resulting in pregnancy. Please,
Mrs. G I hope you understood all these things I told you about Implant insertion.
Mrs. G: Yes Nurse C, thank you very much for your counsel and enlightenment I
am so grateful.
Nurse C: You are welcome.
