2. Report the rating by selecting one of the check boxes as in the table. Put additional information in Remark.

3. Inspector shall give Inspection Report Rating “PENDING” if any FAIL result found in this checking.

#1: Sharp tools should be tethered securely to prevent dropping into products or packaging.

#2: If the sharp tools cannot be tethered, it should be controlled by a responsible person to prevent dropping into products or packaging. Those sharp tools should be dispatched to workers and returned to the responsible person
in daily or task basis (e.g. rework packaging, inspection). The record should be maintained with the following information as minimum:
